Indications

Nexium is indicated in the treatment of heartburn and other symptoms caused by the back flow of stomach acid into the canal of the stomach or the esophagus. A condition known as gastro esophageal reflux disease. It is also prescribed to heal the damage that acid reflux disease can potentially cause.
To heal it is used in combination with the antibiotics Biaxin and Amoxil. Nexium is also used to treat the infection that causes most H. pylori infections and duodenal ulcers (ulcers occurring just beyond the exit from the stomach).

How It Works
Nexium works by reducing the production of stomach acid.

Dosage
Tablets (20 mg or 40 mg)
Initial dosage of Nexium is 20 - 40 mg based on your condition.

Gastro Esophageal Reflux Disease (GERD): For relief of symptoms, the usual dosage is one 20 mg capsule daily for 4 weeks. If symptoms persist, your doctor may prescribe an additional 4 weeks of therapy.
 
Erosive Esophagitis: To heal damage the dosage is 20 or 40 mg of Nexium once daily for 4 to 8 weeks. If you have not fully healed after 8 weeks, your doctor may prescribe an additional 4 to 8 weeks of therapy. To maintain healing the dosage is 20 mg once daily.
 
Duodenal Ulcers: As part of a three drug treatment to rid the body of ulcer-causing H. Pylori bacteria, Nexium is prescribed at a dosage of 40 mg once daily for 10 days. For patients with impaired liver function a dose of 20 mg is recommended.

Nexium comes in delayed release capsules that should be swallowed whole. Be sure to avoid crushing or chewing the capsules. Take Nexium at least one hour before meals.

Precautions
Nexium is contraindicated in patients with a hypersensitivity to esomeprazole. Make sure your doctor knows your medical history, especially if you have any history of liver disease or any type of heart disease. Do not crush, break, or open any of these delayed-release capsules. Swallow this pill whole. It is made to release slowly in the body. Breaking or opening the pill will cause too much of the drug to be released into the body.

You should consult a doctor before using this medication if you are allergic to esomeprazole, other similar drugs or any of the other ingredients in Nexium. If you are having surgery tell your doctor or dentist about all the products you use, prescription, nonprescription and/or herbal.

When taking Esomeprazole you should not take it for more then 4 to 8 weeks (depending on your condition). If Esomeprazole has not rectified the condition it is being used for you should consult your doctor who may recommend an additional period of treatment.

Consult a doctor before taking Nexium if you are pregnant or breastfeeding. Nexium is not expected to cause harm to an unborn baby but you should consult a doctor before taking Nexium if you are pregnant. Your doctor is in the best position to weigh the risks of taking this medicine against the benefits it brings, to determine if it is right for you. Nexium may pass into breast milk.

Drug Interactions
Tell your doctor or pharmacist if you are taking any other prescription or nonprescription, over-the-counter medicine, especially: Blood thinners (such as Clopidogrel or Warfin), Cilostazol, Diazepam, Albendazole, Mebendazole iron salts, Ketoconazole, or HIV drugs (such as Saquinavir). Some drugs or supplements need stomach acid so that the body can absorb them properly (such as iron supplements, calcium supplements, Ampicillin, Dasatinib, and Azole antifungals including Ketoconazole). Esomeprazole decreases stomach acid, and may affect how well these other products work and/or their absorption. Consult your doctor or pharmacist before using esomeprazole in regards to any other medications you take and for advice on how to reduce or avoid negative interactions. There is no problem however, with combining antacids and Nexium and no unwanted interaction will result.
 
If you've been prescribed antibiotics in conjunction with Nexium for the treatment of ulcers be aware that they have occasionally been known to cause severe side effects and life-threatening allergic reactions. If you've been prescribed this combination, be sure to check the entries on Amoxil and Biaxin for more information and or consult with your doctor and or pharmacist.

Esomeprazole may increase the amount of Digoxin that is absorbed into your blood. If you are taking Digoxin you should consult your doctor or pharmacist for details on how these two drugs may interact. Your doctor or pharmacist may recommend adjusting the dosage of on or both medications.

Adverse/Side Effects
It is common that Nexium may cause some of the following minor side effects: Abdominal pain, diarrhea, headache, nausea, stomach pain, gas, constipation or dry mouth. If these side effects persist or become overly bothersome contact your doctor or health-care professional.

If you are experiencing the following rare side effects you should consult your doctor. If these side effects progress to becoming severe contact your doctor immediately. Rare side effects include: abnormal sense of smell, acne, anemia, apathy, back pain, black stools, blood disorders, burping, change in bowel habits, chest pain, chills, confusion, constipation, coughing, cramps, difficulty breathing, difficulty swallowing, dizziness, dry mouth, ear infection, earache, eye infection, facial swelling, fast or irregular heartbeat, fatigue, fever, flu-like symptoms, flushing, frequent or increased urination, fungal infections, general feeling of illness, hernia, hiccups, high blood pressure, hives, hot flushes, impotence, inability to sleep, increased appetite, indigestion, itching, leg and body swelling, loss of appetite, loss of taste, lymph node problems, menstrual problems, migraine, mouth and throat problems, nausea, nervousness, nosebleed, pain, painful joints and muscles, painful urination, prickly or burning sensation, rash, rectal problems, rigidity, ringing in the ears, runny nose, sensitivity to touch, sinus problems, sleep disorders, sleepiness, sore throat, stomach pain, stomach upset, sweating, swelling, taste changes, thirst, tremors, urinary tract infection, vaginitis, vertigo, vomiting, weakness, weight changes, worsening of arthritis, worsening of asthma, worsening of depression.

Call your doctor immediately if you have any of the following serious or worsening symptoms, such as: blood in your urine, enlarged abdomen, enlarged thyroid gland, liver problems, stomach bleeding, vision changes, confusion, abnormal sweating or severe vomiting.

If any of these side effects are worrisome seek medical attention mmediately. A very serious allergic reaction to this drug is rare. Get emergency medical help if you have any of these signs of an allergic reaction: rash, itching/swelling (especially of the face/tongue/throat), severe dizziness, or trouble breathing. This may not be a complete list of side effects and may be subject to change, consult with your doctor or pharmacist if you are experiencing any worrisome side effects.

Overdose
Symptoms of overdose include blurred vision, confusion, drowsiness, dry mouth, flushing, headache, nausea, rapid heartbeat, blurred vision, shortness of breath, tremors, loss of coordination, sweating or seizures. If you suspect overdose, contact your local poison control center or emergency room immediately.
