Pepcid, famotidineChemical NameFamotidine Ingredients Active Ingredients: Famotidine Non-Active Ingredients: cellulose, red and yellow oxide, lactose, polyethylene glycol, sodium stearyl fumarate, titanium dioxide, and other ingredients. Indications Pepcid is used to treat duodenal and gastric ulcers, gastro-esophageal reflux disease which is sometimes called GERD or acid reflux, heartburn and Zollinger-Ellison syndrome. How It Works Famotidine is an anti-ulcer agent that belongs to the group of medications called long acting histamine H2-receptor antagonists. It reduces the amount of acid that the stomach produces. Dosage Tablets (20 mg, 40 mg) Duodenal ulcer: The recommended daily dose of famotidine (Pepcid) is 1 tablet (40 mg) at night. Alternatively, 20 mg every 12 hours may be administered. Treatment should continue for four to eight weeks, but the duration of treatment may be shortened if endoscopy reveals that the ulcer has healed. In most cases of duodenal ulcer healing occurs within four weeks on this regimen. In those patients whose ulcers have not healed completely after four weeks, treatment should be continued for a further four week period. Maintenance Therapy: For the prevention of relapse of duodenal ulceration, it is recommended that therapy with Famotidine be continued with a daily dose of one 20 mg tablet taken at night. However, it should be taken into account that controlled studies have not been extended beyond one year. Benign Gastric Ulcer: The recommended dose is 1 tablet (40 mg of Famotidine) daily, taken at night. Treatment should continue for four to eight weeks but the duration of treatment may be shortened if endoscopy reveals that the ulcer has healed. Maintenance Therapy: For the prevention of recurrence of benign gastric ulcer, the recommended dosage is 20 mg taken at night, which may be given for up to one year. Gastro esophageal Reflux Disease (GERD): The recommended dosage for gastro -esophageal reflux disease is 20 mg of famotidine (Pepcid) taken orally twice daily. For the treatment of esophagitis associated with gastro-esophageal reflux disease, the recommended dosage is 1 tablet (40 mg) twice daily taken orally. If after four to eight weeks of treatment there is no response endoscopic diagnosis should be performed. Zollinger Ellison syndrome: Patients without prior anti-secretory therapy should be started on a dose of 20 mg, every 6 hours. Dosage should be adjusted to individual patient needs and should continue for as long as indicated clinically. Doses up to 800 mg, daily have been used for up to one year without the development of significant adverse effects or tachyphylaxis. Patients who have had prior anti-secretory therapy may be switched directly to Famotidine at a starting dose higher than that recommended for new cases. This starting dose will depend on the severity of the condition and the last dose of the H2-antagonist previously used. Dosage adjustment for patients with moderate or severe renal insufficiency: In adult patients with moderate or severe renal insufficiency, the dose of Famotidine may be reduced to half the dose or the dosing interval may be prolonged to 36 - 48 hours as indicated by the patient's clinical response. If you consider the effect of Famotidine tablets to be too strong or too weak, consult your doctor or pharmacist. Precautions Pepcid is contraindicated in patients allergic or hypersensitive to the effects of famotidine or any of its components, as well as patients with liver disease, kidney disease, stomach cancer, stomach problems, asthma, Chronic obstructive pulmonary disease (COPD), breathing problems, or any condition that would affect the body's absorption or elimination of this drug. If you suffer from any of these conditions you may not be able to take Pepcid or you may require a reduced dosage. Take special care with Famotidine tablets if gastric malignancy has not been excluded by your doctor prior to initiation of therapy of gastric ulcer with Famotidine. Symptomatic response of gastric ulcer to Famotidine therapy does not preclude the presence of gastric malignancy. Caution should be exercised when treating patients with moderate or severe hepatic or renal dysfunction. Since central nervous system (CNS) adverse events have been reported in patients with these conditions. Therapy should always be stopped gradually and as indicated by your doctor so as to prevent relapses. Before taking Pepcid, tell your doctor if you are pregnant or plan to become pregnant during treatment. Pepcid may pass into breast milk and may cause harm to a nursing baby. Tell your doctor right away if you become pregnant. Consult your doctor about the risks and befefits. Drug Interactions Consult a doctor before taking Pepcid, if you are taking any of the following: other medication that reduce acid levels in the stomach. If you experience any of these uncommon but serious side effects contact you doctor immediately: easy bruising/bleeding, fast/slow/irregular heartbeat, persistent nausea/vomiting, signs of infection (fever, persistent sore throat, cough), mental/mood changes (restlessness, confusion, depression, hallucinations), seizure, severe stomach or abdominal pain, dark urine, yellowing skin/eyes. A very serious allergic reaction to this drug is rare. Get emergency medical help if you have any of these signs of an allergic reaction: rash, itching/swelling (especially of the face/tongue/throat), severe dizziness, or trouble breathing.
