FLONASE, fluticasone propionateChemical Namesfluticasone propionate Ingredients Active Ingredients: fluticasone propionate Non-active ingredients: Nasal spray: microcrystalline cellulose and carboxymethylcellulose sodium, dextrose, 0.02% w/w benzalkonium chloride, polysorbate 80, and 0.25% w/w phenylethyl alcohol, and has a pH between 5 and 7. Inhaler: microcrystalline suspension of fluticasone propionate (micronized) in a mixture of 2 chlorofluorocarbon propellants (trichlorofluoromethane and dichlorodifluoromethane) with soya lecithin. Cream: propylene glycol, mineral oil, cetostearyl alcohol, polyoxyl 20 cetostearyl ether, isopropyl myristate, dibasic sodium phosphate, citric acid, purified water and imidurea as a preservative. Indications Nasal spray: Flonase is used to control symptoms of allergic and non-allergic rhinitis, a condition in which the lining of the nose swells and releases fluid that results in a stuffy and runny nose. Inhaler: Flovent propionate is used for the control of asthma in persons requiring continuous treatment. Cream: Cutivate cream and ointment are prescribed for relief of inflamed, itchy rashes and other inflammatory skin conditions. How It Works Fluticasone is a synthetic steroid of the glucocorticoid family of drugs that is used for treating allergic conditions. Fluticasone mimics the naturally-occurring hormone produced by the adrenal glands, cortisol or hydrocortisone. The exact mechanism of action of fluticasone is unknown however, fluticasone has potent anti-inflammatory actions. It is believed that fluticasone exerts its beneficial effects by inhibiting several types of cells and chemicals involved in allergic, immune and inflammatory responses. Dosage Available nasal spray formulations: 50 mcg/spray Available inhaler formulations: 25 mcg, 50 mcg, 100 mcg, 125 mcg, 250 mcg, 500 mcg Available cream formulations: 0.05%, 0.005% Nasal spray: 2 sprays (50 mcg of fluticasone propionate each) in each nostril once daily (total daily dose, 200 mcg). The same dosage divided into 100 mcg given twice daily (e.g., 8 a.m. and 8 p.m.) is also effective. Reduce afterwards. Do not exceed 2 sprays in each nostril (200 mcg) per day. Nasal spray for children: adolescents and children 4 years of age and older should be started with 100 mcg (1 spray in each nostril once daily). Patients not adequately responding to 100 mcg may use 200 mcg (2 sprays in each nostril). Once adequate control is achieved, the dosage should be decreased to 100 mcg (1 spray in each nostril) daily. The maximum total daily dosage should not exceed 2 sprays in each nostril (200 mcg/day). Inhaler: The canister containing the fluticasone propionate should be shaken well before administering. The lungs are emptied of air by breathing out completely. The mouthpiece is placed between the open lips, and the lips are closed firmly around the mouthpiece. The canister's actuating valve is depressed one time while breathing in deeply and slowly. The breath is held for a count of 10, and then normal breathing is resumed. If more than a single inhalation is prescribed, a minute should pass between inhalations. Cream: For treating eczema and other corticosteroid-responsive dermatoses, apply a thin film of Cutivate (fluticasone propionate) cream to the affected skin areas once or twice daily. Rub in gently. Precautions Consult a doctor before taking fluticasone if you are pregnant or breastfeeding, if you are allergic to any of its ingredients, or if you have (or have a history of) tuberculosis, active infections, ocular herpes, decreased adrenal gland function, glaucoma, chickenpox, measles, psoriasis, or any allergies (especially to nuts). Do not suddenly stop or alter a fluticasone regimen without consulting a doctor, as adrenal insufficiency may result. Inhalant: The inhalant form of fluticasone contains trichlorofluoromethane and dichlorodifluoromethane, substances that harm public health and environment by destroying ozone in the upper atmosphere. Nasal spray: Cream: Avoid prolonged application to the face since the face, more than other areas of the body, may exhibit atrophic changes after prolonged treatment with potent topical corticosteroids. This must be borne in mind when treating such conditions as psoriasis, discoid lupus erythematosus and severe eczema. Do not use Cutivate cream on the face, underarms, or groin areas unless your doctor tells you to do so. Do not apply Cutivate if you have an infection in the affected skin area. Tell your doctor if you have had any other problems in that area. Do not use Cutivate to treat rosacea or skin inflammation near the mouth. Also avoid using Cutivate on thin, weakened skin that has lost its elasticity. The treated skin area should not be bandaged, covered, or wrapped unless you have been directed to do so by your doctor. If used in psoriasis, careful patient supervision is important. Topical steroids may be hazardous in psoriasis for a number of reasons, including rebound relapses, development of tolerances, risk of generalized pustular psoriasis and development of local or systemic toxicity due to impaired barrier function of the skin. The preparation also contraindicated in the treatment of rosacea, acne vulgaris, perioral dermatitis, primary cutaneous viral infections (i.e., herpes simplex, chickenpox), perianal and genital pruritus, primarily infected skin lesions caused by infection with fungi or bacteria and dermatoses in children, including dermatitis and diaper rash.
