Indications Symmetrel is indicated in the prophylaxis and treatment of symptoms of influenza caused by Type A influenza virus, especially in patients at high risk where vaccination is not indicated, as well as in the treatment of Parkinson's disease and drug-induced extrapyramidal reactions. How It Works Amantadine is an anti-viral agent. The mechanism of action of amantadine appears to be the inhibition of viral penetration into the host cell, with which it prevents the replication and spread of the viral infection. Dosage Available capsule sizes: 100 mg Do not stop taking Symmetrel without consulting a doctor. Influenza A Virus: The standard adult dosage of Symmetrel is 200 mg daily in a single dose. In healthy adults, a single daily dose of 100 mg has been shown to be effective as a prophylactic against influenza A virus, and is recommended for healthy adults, patients 65 and older, and patients who have shown intolerance to 200 mg doses of Symmetrel. In patients one to nine years old, the total daily dose should be calculated on the basis of 2 to 4 mg/lb/day (4.4 to 8.8 mg/kg/day), but not to exceed 150 mg per day. In patients 9 to 12 years old, The total daily dose is 200 mg given as one tablet of 100 mg twice a day. The 100 mg daily dose has not been studied in this pediatric population. Therefore, there are no data which demonstrate that this dose is as effective as or is safer than the 200 mg daily dose in this patient population. Prophylactic dosing should be started in anticipation of an influenza A outbreak and before or after contact with individuals with influenza A virus respiratory tract illness. Symmetrel should be continued daily for at least 10 days following a known exposure. If Symmetrel is used chemoprophylactically in conjunction with inactivated influenza A virus vaccine until protective antibody responses develop, then it should be administered for 2 to 4 weeks after the vaccine has been given. When inactivated influenza A virus vaccine is unavailable or contraindicated, SYMMETREL should be administered for the duration of known influenza A in the community because of repeated and unknown exposure. Because Symmetrel does not completely prevent the host immune response to influenza A infection, individuals who take this drug may still develop immune responses to natural disease or vaccination and may be protected when later exposed to antigenically related viruses. Following vaccination during an influenza A outbreak, Symmetrel prophylaxis should be considered for the 2- to 4-week time period required to develop an antibody response. Parkinson’s The standard dosage of Symmetrel in treating Parkinson’s disease is 100 mg twice daily, if Symmetrel is the sole Parkinson’s medication being taken. An initial dose of 100 mg once daily is recommended, and if well tolerated for several weeks, dosage may be increased to 200 mg daily (or up to a maximum of 400 mg daily). Patients initially benefitting from Symmetrel sometimes experience a drop-off of effectiveness after a few months. Benefit may be regained by increasing the dose to 300 mg daily, or temporarily discontinuing Symmetrel for several weeks, followed by reinitiating the drug. A decision to use other antiparkinson drugs may be necessary. Notify your doctor if you develop: slurred speech, shortness of breath, swelling of the ankles/feet, unusual fatigue, vision disturbances, difficulty urinating, skin rash, mental/mood changes (sometimes severe, including rare thoughts of suicide), muscle stiffness, uncontrolled muscle movements, unusual sweating, fast heartbeat, unexplained fever. Overdose Symptoms of overdose include nausea, vomiting, slurred speech, blurred vision, drowsiness, memory loss, depression, excessive excitability, increased sensitivity to the sun, loss of consciousness, fast or irregular heartbeat.. If you suspect overdose, contact your local poison control center or emergency room immediately. Deaths have been reported from overdose with Symmetrel. The lowest reported acute lethal dose was 1 gram. Acute toxicity may be attributable to the anticholinergic effects of amantadine. Drug overdose has resulted in cardiac, respiratory, renal or central nervous system toxicity.
