It is also used to treat a chronic pain disorder called fibromyalgia, and to treat pain caused by nerve damage in people with diabetes (Diabetic Peripheral Neuropathic Pain) How It Works Duloxetine is an antidepressant in a group of drugs called selective serotonin and norepinephrine re-uptake inhibitors (SSNRIs). Duloxetine affects chemicals in the brain that may become unbalanced and cause depression. Dosage Available in: 20 mg, 30 mg, 40 mg, or 60 mg capsules The dosage for Cymbalta should be 40 mg/day (taken as 20 mg twice daily) up to 60 mg/day. Some patients may want to start at a lower dosage of 30 mg/day for a week and allow their bodies to adjust to the medication as they work up to 40-60 mg/day dose. Precautions Consult a doctor before taking Cymbalta if you are pregnant or breastfeeding. Duloxetine may be harmful to an unborn baby, and should not be used, especially towards the end of pregnancy. Tell your doctor if you are pregnant or plan to become pregnant during treatment.Duloxetine can pass into breast milk and may be harmful to a baby. Do not use this medication without telling your doctor if you are breast-feeding a baby. Do not use this medication if you are allergic to duloxetine, or if you have untreated or uncontrolled glaucoma. If you are younger then 24 years old there is an increased possibility of having suicidal thoughts when you first start taking antidepressants. Family and friends should be aware of potential changes in your mood or other alert symptoms. Your doctor should check with you at regular intervals for the first 12 weeks. Drug Interactions
Do not start, stop, or change the dosage of Cymbalta medicine before checking with your doctor. Certain medications taken with this product could result in serious drug interactions. Do not take MAO inhibitors (Furazolidone, Isocarboxazid, Linezolid, Moclobemide, Phenelzine, Procarbazine, Rasagiline, Selegiline, Tranylcypromine) before treatment, during treatment, or for at least a week after finishing treatment of Duloxetine. This drug should also not be used with the following medications because very serious (possibly fatal) interactions may occur: Fluvoxamine, Sibutramine, Thioridazine, Tryptophan. If you are currently using these medications tell your doctor or pharmacist before starting duloxetine. Before using this medication, tell your doctor or pharmacist of all prescription and nonprescription/herbal products you may use, especially: drugs removed from your body by the liver: (Cimetidine, Antipsychotics such as Perphenazine, antiarrhythmics such as Propafenone/Flecainide/Quinidine, certain quinolone antibiotics such as Ciprofloxacin, antidepressants such as Desipramine/Fluoxetine /Imipramine/Paroxetine), drugs to treat high blood pressure (Clonidine, Guanethidine), "water pills" (diuretics), drugs that can cause bleeding/bruising (Aspirin, antiplatelet drugs such as Clopidogrel, NSAIDs such as Ibuprofen, "blood thinners" such as Heparin/Warfarin). Talk to your doctor before taking any medicine for pain, arthritis, fever, or swelling. This includes aspirin, ibuprofen (Advil, Motrin), naproxen (Aleve, Naprosyn), Diclofenac (Voltaren), Indomethacin, Piroxicam (Feldene), Nabumetone (Relafen), Etodolac (Lodine), and others. Taking any of these drugs with Duloxetine may cause you to bruise or bleed easily. If your doctor has directed you to take low-dose aspirin for heart attack or stroke prevention you should continue taking it unless your doctor instructs you not to. Adverse/Side Effects If you have any signs of an allergic reaction: skin rash or hives; difficulty breathing; swelling of your face, lips, tongue, or throat contact your doctor immediately. Call your doctor at once if you have any of these serious side effects: nausea, stomach pain, low fever, loss of appetite, dark urine, clay-colored stools, jaundice (yellowing of the skin or eyes); painful or difficult urination; easy bruising or bleeding, nosebleeds; black, bloody, or tarry stools; very stiff (rigid) muscles, high fever, vomiting, diarrhea, sweating, fast or uneven heartbeats, tremors, overactive reflexes; headache, trouble concentrating, memory problems, confusion, hallucinations; or weakness, feeling unsteady, loss of coordination, fainting, seizure, shallow breathing or breathing that stops. Less serious side effects that will usually clear up on their own may include: dry mouth, blurred vision; drowsiness, dizziness, spinning sensation; mild nausea, constipation, gas; sleep problems (insomnia); joint or muscle pain; weight changes; or decreased sex drive, impotence, or difficulty having an orgasm.
