Ingredients
Active Ingredients: amitryptaline
Non-Active Ingredients: calcium phosphate, cellulose, colloidal silicon dioxide, hydroxypropyl cellulose, hydroxypropyl methylcellulose, lactose, magnesium stearate, starch, stearic acid, talc, and titanium dioxide. 10 mg amitriptyline HCl tablets also contain FD&C Blue 1. 25 mg amitriptyline HCl tablets also contain D&C Yellow 10, FD&C Blue 1, and FD&C Yellow 6. 50 mg amitriptyline HCl tablets also contain D&C Yellow 10, FD&C Yellow 6 and iron oxide. 75 mg amitriptyline HCl tablets also contain FD&C Yellow 6. 100 mg amitriptyline HCl tablets also contain FD&C Blue 2 and FD&C Red 40. 150 mg amitriptyline HCl tablets also contain FD&C Blue 2 and FD&C Yellow 6.

Indications
Elavil is indicated in the treatment of major depression, pain from ankylosing spondylitis, and migraines.

How It Works
Amitryptiline is a tricyclic antidepressant, an antidepressant that functions primarily as a serotonin-norepinephrine reuptake inhibitor (SNRI). It increases blood levels of serotonin and norepinephrine, two neurotransmitters that tend to regulate and stimulate positive feelings.

Dosage
Available tablet sizes: 10 mg, 25 mg, 50 mg, 75 mg

Dosage should be initiated at a low level and increased gradually, noting carefully the clinical response and any evidence of intolerance.

For adult outpatients 75 mg of amitriptyline HCl a day in divided doses is usually satisfactory. If necessary, this may be increased to a total of 150 mg per day. Increases are made preferably in the late afternoon and/or bedtime doses. A sedative effect may be apparent before the antidepressant effect is noted, but an adequate therapeutic effect may take as long as 30 days to develop.

The usual maintenance dosage of amitriptyline HCl is 50 to 100 mg per day. In some patients 40 mg per day is sufficient.

An alternate method of initiating therapy in outpatients is to begin with 50 to 100 mg amitriptyline HCl at bedtime. This may be increased by 25 or 50 mg as necessary in the bedtime dose to a total of 150 mg per day.

Hospitalized patients may require 100 mg a day initially. This can be increased gradually to 200 mg a day if necessary. A small number of hospitalized patients may need as much as 300 mg a day.

In general, lower dosages are recommended for elderly and adolescent patients. 10 mg three times a day with 20 mg at bedtime may be satisfactory in adolescent and elderly patients who do not tolerate higher dosages.
 
 
Precautions
Consult a doctor before taking Elavil if you are pregnant or breastfeeding, if you are allergic to any of its ingredients, or if you have a history of schizophrenia, paranoia, bipolar disorder, suicidality, impaired liver function, or diabetes.

Inform your doctor if you are taking Elavil and are due to undergo surgery or electroshock therapy.

Roughly 10% of Caucasians are “poor metabolizers” of tricyclic antidepressants. Poor metabolizers have higher than expected plasma concentrations of tricyclic antidepressants (TCAs) when given usual doses.

Drug Interactions
Consult a doctor before taking Elavil if you are taking monoamine oxidase inhibitors (MAOIs), quinidine, cimetidine, other antidepressants, phenothiazines, propafenone, flecainide, guanethidine, thyroid medication, alcohol, barbiturates, depressants, disulfiram, or any anticholinergic agents (including epinephrine).

Adverse/Side Effects
Troublesome side effects of Elavil include fast heart rate, blurred vision, urinary retention, dry mouth, constipation, weight gain or loss, and low blood pressure on standing. Rash, hives, seizures, and hepatitis are rare side effects.

Amitriptyline should be used with caution in patients with seizures since it can increase the risk of seizures. Amitriptyline is used with caution in patients with prostate enlargement because of the risk of increasing retention of urine due to the inability to urinate. Amitriptyline can cause elevated pressure in the eyes of certain patients with glaucoma.

Antidepressants increased the risk of suicidal thinking and behavior in short-term studies in children and adolescents with depression and other psychiatric disorders. Anyone considering the use of amitriptyline or any other antidepressant in a child or adolescent must balance this risk with the clinical need. Patients who are started on therapy should be closely observed for clinical worsening, suicidal thinking or behavior, and unusual changes in behavior.

If amitriptyline is discontinued abruptly, dizziness, headache, nausea, and restlessness may occur. Withdrawal symptoms may occur when even a few doses are missed. Therefore, it is recommended that the dose of antidepressant be reduced gradually when therapy is discontinued.

Overdose
Symptoms of overdose include impaired myocardial contractility, confusion, disturbed concentration, transient visual hallucinations, dilated pupils, disorders of ocular motility, agitation, hyperactive reflexes, polyradiculoneuropathy, stupor, drowsiness, muscle rigidity, vomiting, hypothermia, hyperpyrexia, or any of the effects listed in Adverse/Side Effects. If you suspect overdose, contact your local poison control center or emergency room immediately.
