Luvox, fluvoxamine

Chemical Name
Fluvoxamine

Ingredients
Active Ingredients: Fluvoxamine

Non-Active Ingredients: Carnauba wax, hydroxypropyl methylcellulose, mannitol, polyethylene glycol, polysorbate 80, pregelatinized starch (potato), silicon dioxide, sodium stearyl fumarate, starch (corn), and titanium dioxide. The 50 mg and 100 mg tablets also contain synthetic iron oxides.

Indications
Luvox is used to treat obsessive-compulsive disorder, which is characterized by recurrent and persistent ideas, thoughts, impulses or images (obsessions) that are ego-dystonic and/or repetitive, purposeful, and intentional behaviors (compulsions) that are recognized by the person as excessive or unreasonable.

How It Works
The mechanism of action of fluvoxamine maleate in obsessive-compulsive disorder is presumed to be linked to its specific targeting of serotonin reuptake inhibition in brain neurons.

Dosage
Tablets (50 mg, 100 mg)

Adults:
The recommended starting dose for Luvox tablets in adult patients is 50 mg, administered as a single daily dose at bedtime. In the controlled clinical trials establishing the effectiveness of Luvox tablets in OCD, patients were titrated within a dose range of 100 to 300 mg/day. Consequently, the dose should be increased in 50 mg increments every 4 to 7 days, as tolerated, until maximum therapeutic benefit is achieved, not to exceed 300 mg per day.

Pediatric:
The recommended starting dose for Luvox tablets in pediatric populations (ages 8-17 years) is 25 mg, administered as a single daily dose at bedtime. In a controlled clinical trial establishing the effectiveness of Luvox Tablets in OCD, pediatric patients (ages 8-17) were titrated within a dose range of 50 to 200 mg/day.

At least 14 days should elapse between discontinuation of an MAOI and initiation of therapy with Luvox.

Precautions
Luvox is contraindicated in people who have hypersensitivity to ingredients in Luvox, patients with unstable epilepsy or with controlled epilepsy, co-administration of tizanidine, thioridazine, alosetron, or pimozide with Luvox tablets is contraindicated. The use of MAOI's comitantly with or within 14 days of treatment with Luvox is not recommended.

Speak to a doctor if you have the following conditions: there is a chance that conditions might worsen in patients with major depressive disorder (MDD) when taking Luvox, there is a chance of developing suicidal thoughts, that is why it is important to be monitored by a doctor while on this medication.

Be monitored appropriately and observed closely for clinical worsening, suicidality, and unusual changes in behavior, especially during the initial few months.  The following symptoms might be a sign of worsening condition: anxiety, agitation, panic attacks, insomnia, irritability, hostility, aggressiveness, impulsivity, akathisia (psychomotor restlessness), hypomania, and mania.

People should be alerted about the need to monitor patients for the emergence of agitation, irritability, unusual changes in behavior, and the other symptoms described above, as well as the emergence of suicidality,
Patients with liver dysfunction should begin with a low dose of Luvox tablets.

Taking Luvox and monoamine oxidase inhibitors (MAOIs) can have fatal consequences. Speak with doctor if you are currently on MAOIs, symptoms may include:  Overheating of the body, autonomic instability with possible rapid fluctuations of vital signs, rigidity and mental status changes which can progress to delirium and coma.

Therefore, Luvox tablets should not be used in combination with an MAOI, or within 14 days of discontinuing treatment with an MAOI. Similarly, at least 14 days should be allowed after stopping Luvox before beginning treatment with monoamine oxidase inhibitors.

Thioridazine administration with Luvox can cause abnormal heart rhythms.

Taking tizanidine, or fluvoxamine concurrently with Luvox may raise blood pressure. Pimozide interaction may cause an abnormally rapid heart rhythm that originates from a ventricle, one of the lower chambers of the heart. Patients should be aware of the possibility of getting serotonin syndrome. The excess serotonin activity produces a spectrum of specific symptoms including cognitive, autonomic, and somatic effects. Drug Interactions
Consult a doctor before taking Luvox, if you are taking: Carbamazepine, lithium, Monoamine Oxidase Inhibitors (MAOs), Methadone, Pimozide, Ramelteon, Tacrine, Thioridazine, Tizanidine, Tricyclic Antidepressants (TCAs), Triptans, SSRI (fluoxetine, fluvoxamine, paroxetine, sertraline), Tryptophan, Alosetron, Digoxin, Diltiazem, Mexiletine, Propranolol and other beta-blockers, Theophylline, Warfarin and other drugs that interfere with hemostasis (NSAIDs, Aspirin)

Before using Luvox, tell your doctor or pharmacist about all prescription and nonprescription medications, including herbal products, minerals and vitamins you may be taking. Other medications may affect how Luvox works or may cause other unforeseen complications.

Adverse/Side Effects
It is common that Luvox causes the following minor side effects: nausea, somnolence, insomnia, asthenia, nervousness, dyspepsia, abnormal ejaculation, sweating, anorexia, tremors, malaise, partial or complete loss of consciousness and vomiting. If these side effects persist or become overly bothersome contact your doctor or health-care professional.

Infrequent side effects: anorgasmia, decreased libido, dry mouth, rhinitis, taste perversion, changes in urinary frequency, agitation, depression, dysmenorrhea, flatulence, hyperkinesia, rash, weakness, abnormal ejaculation (mostly delayed ejaculation), anxiety, rhinitis, anorgasmia (in males), depression, libido decreased, pharyngitis, agitation, impotence, myoclonus/twitch, thirst, weight loss, leg cramps, myalgia, and urinary retention, uterine pain during menstruation, skin discoloration, black pigment of stool, vomit, anemia, overactive restlessness, bruising, manic reaction, abnormal dreams, akathisia, convulsion, dyskinesia, dystonia, euphoria, extra pyramidal syndrome, twitching, nosebleed, spitting, sudden laryngeal spasm, hives, blood in urine, or excessive vaginal bleeding.

Rare side effects: bloody vomit, withdrawal syndrome, blood in semen, and coughing up blood.

If any of these side effects become bothersome seek medical attention immediately. A very serious allergic reaction to this drug is rare. Get emergency medical help if you have any of these signs of an allergic reaction: rash, itching/swelling (especially of the face/tongue/throat), severe dizziness, or trouble breathing. This may not be a complete list of side effects and may be subject to change, consult with your doctor or pharmacist if you are experiencing any worrisome side effects.

Overdose
If you suspect overdose, contact your local poison control center or emergency room immediately.
