Chemical Names

Ingredients
Active Ingredients: Doxepin
Non-Active Ingredients: Hard gelatin capsules (which may contain Blue 1, Red 3, Red 40, Yellow 10, and other inert ingredients); magnesium stearate; sodium lauryl sulfate; starch.

Indications
Sinequan is indicated in the treatment of depression and/or anxiety associated with alcoholism, psychiatric conditions, or manic-depressive conditions.

How It Works
Doxepin is in a group of drugs called tricyclic antidepressants. Doxepin affects chemicals in the brain that may become unbalanced.

Dosage
Tablets (10 mg 25 mg, 75 mg)

A starting daily dose of 75 mg is recommended. Dosage may subsequently be increased or decreased at appropriate intervals and according to individual response. The usual optimum dose range is 75 mg/day to 150 mg/day.

In more severely ill patients higher doses may be required with subsequent gradual increase to 300 mg/day if necessary. Additional therapeutic effect is rarely to be obtained by exceeding a dose of 300 mg/day.

Precautions
Do not use this medication if you are allergic to Doxepin, or any of its other ingredients, or if you have glaucoma or problems with urination. You may have thoughts about suicide when you first start taking an antidepressant, especially if you are younger than 24 years old. Your doctor will need to schedule regular visits for at least the first 12 weeks of treatment. Call your doctor at once if you have any new or worsening symptoms such as: mood or behavior changes, anxiety, panic attacks, trouble sleeping, or if you feel impulsive, irritable, agitated, hostile, aggressive, restless, hyperactive (mentally or physically), more depressed, or have thoughts about suicide or hurting yourself. Before taking Doxepin, tell your doctor if you are allergic to any drugs, or if you have: bipolar disorder (manic-depression); or diabetes (Doxepin may raise or lower blood sugar).

If you have any of these conditions, you may need a dose adjustment or special tests to safely take this medication. This medication may be harmful to an unborn baby. Tell your doctor if you are pregnant or plan on becoming pregnant during treatment. It is not known whether Doxepin passes into breast milk or if it could harm a nursing baby. Do not use this medication without telling your doctor if you are breast-feeding a baby.
 

Drug Interactions
Consult a doctor before taking Sinequan if you are taking any other anti-depressants or antipyschotic medications. Do not use Doxepin if you have used a MAO inhibitor such as isocarboxazid (Marplan), phenelzine (Nardil), rasagiline (Azilect), selegiline (Eldepryl, Emsam), or tranylcypromine (Parnate) within the past 14 days. Serious, life-threatening side effects can occur if you take Doxepin before the MAO inhibitor has cleared from your body.

Before taking Doxepin, tell your doctor if you have used an "SSRI" antidepressant in the past 5 weeks, such as Citalopram (Celexa), Escitalopram (Lexapro), Fluoxetine (Prozac, Sarafem), Fluvoxamine (Luvox), Paroxetine (Paxil), or Sertraline (Zoloft). Before taking Doxepin, tell your doctor if you are currently using any of the following drugs: Cimetidine (Tagamet) or heart rhythm medications such as Flecainide (Tambocor), Propafenone (Rhythmol), or Quinidine (Cardioquin, Quinidex, Quinaglute).

The effects of some drugs can change if you take other drugs or herbal products at the same time. This can increase your risk for serious side effects or may cause your medications not to work correctly tell your doctor about any and all products you take. While taking Sinequan, do not start, stop, or change the dosage without your doctor's approval.

Some other products that may interact with this drug include: Arbutamine, Sibutramine, bronchodilators/decongestants (such as Albuterol, Epinephrine, Phenylephrine), stimulants (such as Amphetamines, Methylphenidate, streets drugs including MDMA/"ecstasy"), thyroid supplements, anticholinergic drugs (such as Benztropine, Belladonna alkaloids), central-acting drugs to treat high blood pressure (such as Clonidine, Guanabenz, Reserpine).

Other medications can affect the removal of Doxepin from your body, thereby affecting how Doxepin works. These drugs include Cimetidine, Fluconazole, St. John's Wort, Terbinafine, drugs to treat irregular heart rate (such as Quinidine/Propafenone/flecainide), and antidepressants (such as SSRIs including Paroxetine/Fluoxetine/Fluvoxamine).

Adverse/Side Effects
Get emergency medical help if you experience signs of an allergic reaction: hives, difficulty breathing, swelling of your face, lips, tongue, or throat.
Call your doctor at once if you have any new or worsening symptoms such as: mood or behavior changes, anxiety, panic attacks, trouble sleeping, or if you feel impulsive, irritable, agitated, hostile, aggressive, restless, hyperactive (mentally or physically), more depressed, or have thoughts about suicide or hurting yourself.

Call your doctor at once if you have any of these serious side effects: fast, pounding, or uneven heart rate, confusion, hallucinations, or seizure (convulsions), easy bruising or bleeding, unusual weakness, feeling light-headed, fainting, restless muscle movements in your eyes, tongue, jaw, or neck, tremors or uncontrollable shaking, urinating less than usual or not at all, or extreme thirst with headache, nausea, vomiting, and weakness.

Less serious side effects that should go away on their own include: nausea, vomiting, constipation, loss of appetite, dry mouth, weight changes; weakness, lack of coordination, numbness or tingly feeling; feeling dizzy, drowsy, or tired, sleep problems (insomnia), nightmares; blurred vision, headache, ringing in your ears, increased sweating, breast swelling (in men or women), or decreased sex drive, impotence, or difficulty having an orgasm.

Overdose
Symptoms of overdose include extreme drowsiness, hallucinations, fast/irregular heartbeat, fainting, slow/shallow breathing, seizures. If you suspect overdose, contact your local poison control center or emergency room immediately.

Storage Instructions
Store at room temperature between 68-77 degrees F (20-25 degrees C) away from light and moisture. Do not store in the bathroom. Keep all medicines away from children and pets.
