How It Works Imipramine, a tertiary amine, affects numerous neurotransmitter systems known to be involved in the etiology of depression. Dosage Tablets (25mg, 75 mg) Adult Dose Hospitalized Patients - Initially, 100mg/day in divided doses gradually increased to 200mg/day as required. If no response after two weeks, increase to 250 to 300mg/day. Outpatients - Initially, 75mg/day increased to150mg/day. Dosages over 200mg/day are not recommended. Maintenance: 50 to 150mg/day. Adolescent and Geriatric Patients - Initially, 30 to 40mg/day; it is generally not necessary to exceed 100mg/day. For patients with urination control problems: Initially, an oral dose of 25mg/day should be tried in children aged six and older. Medication should be given one hour before bedtime. If a satisfactory response does not occur within one week, increase the dose to 50mg nightly in children younger than 12 years of age; children over 12 may receive up to 75mg nightly. A dose of 2.5mg/kg/day should not be exceeded. Precautions Tofranil is contraindicated in people who have hypersensitivity to ingredients in Tofranil. Avoid excessive exposure to sunlight while on this medication, as you will become more photosensitive. Both elevation and lowering of blood sugar has been noted in patients taking this medication. Tofranil should be used with caution in patients with both kidney and liver problems. Patients who develop a fever and a sore throat during therapy with imipramine hydrochloride should have leukocyte and differential blood counts performed. Safety and effectiveness in pediatric patients has not been established. Avoid drinking alcohol. It can cause dangerous side effects when taken together with imipramine. Families and caregivers of patients being treated with antidepressants for major depressive disorder or other indications, both psychiatric and non-psychiatric, should carefully observe patients for the emergence of agitation, irritability, and unusual changes in behavior. Taking this drug can result in heart attack or other heart problems that might be fatal, therefore, extreme caution must be exercised when taking this medication. For people who are prone to seizure attack, this medication may lower the seizure threshold, making these types of patients more prone to seizure attacks. Guanethidine, clonidine, or similar agents may actually block the effects of this drug. Speak to a doctor if you are taking these medications. Tofranil may enhance the CNS depressant effects of alcohol. This medication impairs your performance. Exercise caution if working in a hazardous profession. Alcohol, barbiturates, benzodiazepines, e.g. lorazepam (Ativan), diazepam (Valium), temazepam (Restoril), oxazepam (Serax), clonazepam (Klonopin), zolpidem (Ambien), Reserpineand narcotics, isocarboxazid (Marplan), phenelzine (Nardil), tranylcypromine (Parnate), and procarbazine (Matulane), cimetidine (Tagamet) can increase imipramine blood levels by reducing elimination of imipramine from the body and possibly lead to imipramine-related side effects. Other drugs which share this effect include propafenone (Rythmol), flecainide (Tonocard), quinidine (Quinidex, Quinaglute), and fluoxetine (Prozac). Before using this Tofranil, tell your doctor or pharmacist about all prescription and non-prescription medications, including herbal products, minerals and vitamins you may be taking. Other medications may affect how Tofranil works or may cause other unforeseen complications. Adverse/Side Effects It is common that Tofranil causes the following minor side effects: nausea, vomiting, stomach pain, loss of appetite, constipation or diarrhea, dry mouth, unpleasant taste, weight changes, weakness, lack of coordination, feeling dizzy, drowsy, or tired, nightmares, blurred vision, headache, ringing in your ears, breast swelling (in men or women), decreased sex drive, impotence, or difficulty having an orgasm.
