Flovent, fluticasone propionateChemical NamesFluticasone propionate Ingredients Active Ingredients: fluticasone propionate Non-active ingredients: Nasal spray: microcrystalline cellulose and carboxymethylcellulose sodium, dextrose, 0.02% w/w benzalkonium chloride, polysorbate 80, and 0.25% w/w phenylethyl alcohol, and has a pH between 5 and 7. How It Works Fluticasone is a synthetic steroid of the glucocorticoid family of drugs that is used for treating allergic conditions. Fluticasone mimics the naturally-occurring hormone produced by the adrenal glands, cortisol or hydrocortisone. The exact mechanism of action of fluticasone is unknown however, fluticasone has potent anti-inflammatory actions. It is believed that fluticasone exerts its beneficial effects by inhibiting several types of cells and chemicals involved in allergic, immune and inflammatory responses. Dosage Available nasal spray formulations: 50 mcg/spray Available inhaler formulations: 25 mcg, 50 mcg, 100 mcg, 125 mcg, 250 mcg, 500 mcg Available cream formulations: 0.05%, 0.005% Nasal spray: 2 sprays (50 mcg of fluticasone propionate each) in each nostril once daily (total daily dose, 200 mcg). The same dosage divided into 100 mcg given twice daily (e.g., 8 a.m. and 8 p.m.) is also effective. Reduce afterwards. Do not exceed 2 sprays in each nostril (200 mcg) per day. Nasal spray for children: adolescents and children 4 years of age and older should be started with 100 mcg (1 spray in each nostril once daily). Do not use Cutivate cream on the face, underarms, or groin areas unless your doctor tells you to do so. Do not apply Cutivate if you have an infection in the affected skin area. Also avoid using Cutivate on thin, weakened skin that has lost its elasticity. The treated skin area should not be bandaged, covered, or wrapped unless you have been directed to do so by your doctor. If used in psoriasis, careful patient supervision is important. Topical steroids may be hazardous in psoriasis for a number of reasons, including rebound relapses, development of tolerances, risk of generalized pustular psoriasis and development of local or systemic toxicity due to impaired barrier function of the skin. The preparation also contraindicated in the treatment of rosacea, acne vulgaris, perioral dermatitis, primary cutaneous viral infections (i.e., herpes simplex, chickenpox), perianal and genital pruritus, primarily infected skin lesions caused by infection with fungi or bacteria and dermatoses in children, including dermatitis and diaper rash. Drug Interactions Inhaler and nasal spray: ritonavir and ketoconazole (and any other cytochrome P450 3A4 inhibitors) can significantly increase plasma fluticasone propionate exposure, resulting in significantly reduced serum cortisol concentrations. As well, other steroids should not be used in conjunction with fluticasone. Cream: No interactions with food or other drugs have been reported, however that does not mean none exist. You should disclose any medication and speak to your doctor before taking any medication. Adverse/Side effects Nasal spray: The most common side effects associated with fluticasone are headache, throat infection, nasal irritation, sneezing, cough, nausea, vomiting, and nosebleeds. Hypersensitivity reactions such as skin rash, itching, facial swelling, and anaphylaxis may occur. Some children may experience growth suppression when using fluticasone. Inhaler: Common side effects: mild wheezing, mild cough, ear nose and throat problems, pharyngitis, nasal congestion, sinusitis, nasal discharge, dysphonia, allergic rhinitis, oral candidiasis, upper respiratory infection, headache. Less common side effects: Oral candidiasis or thrush (fungal infection). The risk for candidiasis in children is lower than in adults. Using a spacer and washing the mouth out with water following each use reduces the risk of candidiasis. Hoarseness, weakened bones and fractures may also occur. Rare cases: glaucoma, cataracts, immediate and delayed hypersensitivity reactions, including urticaria and rash and other rare events of angioedema and bronchospasm, have been reported. Cream and ointment: Side effects cannot be anticipated. If any develop or change in intensity, inform your doctor as soon as possible. Side effects of Cutivate may include: pruritus, burning, hypertrichosis, increased erythema, hives, irritation, and lightheadedness, dryness, infected or worsening eczema, itchy spots, rash, skin irritation, stinging, irritation, folliculitis, acneiform eruptions, hypopigmentation, perioral dermatitis, allergic contact dermatitis, secondary infection, skin atrophy, striae, dilation of the superficial blood vessels and miliaria. Also, there are reports of the development of pustular psoriasis from chronic plaque psoriasis following reduction or discontinuation of potent topical steroid roducts. Warnings Inhaler: After withdrawal from systemic corticosteroids, a number of months are required for recovery of HPA function. Patients who have been previously maintained on 20 mg or more per day of prednisone (or its equivalent) may be most susceptible, particularly when their systemic corticosteroids have been almost completely withdrawn. During periods of stress or a severe asthma attack, patients who have been withdrawn from systemic corticosteroids should resume oral corticosteroids use. If bronchospasm occurs following dosing with Flovent Inhalation Aerosol, it should be treated immediately with a fast-acting inhaled bronchodilator. It is possible that systemic corticosteroid effects such as hypercorticism and adrenal suppression may manifest in patients.
