Rhinocort, budesonideChemical Names
budesonide
Ingredients
Active ingredients: budesonide
Non active: micronized suspension of budesonide in an aqueous medium. Microcrystalline cellulose and carboxymethyl cellulose sodium, dextrose anhydrous, polysorbate 80, disodium edetate, potassium sorbate, and purified water are contained in this medium; hydrochloric acid is added to adjust the pH to a target of 4.5.
Indications
The budesonide inhaler is used for the control of asthma in persons requiring continuous, prolonged treatment.
How It Works
Budesonide is a man-made glucocorticoid steroid related to the naturally-occurring hormone produced in the adrenal glands. It is used for treating asthma by inhalation. Glucocorticoid steroids such as cortisol or budesonide have potent anti-inflammatory actions that reduces inflammation and hyper-reactivity (spasm) of the airways caused by asthma.
Dosage
Available nasal spray formats: 50 mcg, 64 mcg, 100 mcg
The recommended starting dose for adults and children 6 years of age and older is 64 mcg per day, administered as one 32 mcg spray per nostril once daily.
The maximum recommended dose for adults (12 years of age and older) is 256 mcg per day administered as four 32 mcg sprays per nostril once daily.
The maximum recommended dose for pediatric patients older than 12 years of age is 128 mcg per day administered as two 32 mcg sprays per nostril once daily.
It is always desirable to titrate an individual patient to the minimum effective dose to reduce the possibility of side effects.
Prior to initial use, the container must be shaken gently and the pump must be primed by actuating eight times. If not used for more than 14 days, rinse the applicator and re-prime with two sprays or until a fine spray appears.
Precautions
Tell your doctor immediately if your medical history consists of: thyroid problems, liver disease, infection, recent nasal surgery or trauma, allergies (especially drug allergies). This may have undesirable consequences that may require additional corticosteroid treatment, children who have had serious medical problems such as serious infections, injuries or surgeries. This precaution applies for up to one year after stopping use of this drug.
Caution is advised when using this drug in children. Though it is unlikely to occur with intranasal steroids such as budesonide, this medication may temporarily slow down a child's rate of growth, but it will probably not affect final adult height. Monitor your child's height periodically.
This medication should be used only when clearly needed during pregnancy. If you are pregnant or thinking of becoming pregnant consult a doctor before taking this medication.
Drug Interactions
Ketoconazole (Nizoral, Extina, Xolegel, Kuric) increases the concentrations in blood of budesonide, and this may lead to an increase in the side effects of budesonide. No drug interactions have been described with inhaled budesonide.
Tell your doctor of any over-the-counter or prescription medication you may take, especially of: other oral corticosteroids, immunosuppressants, all allergy medications, drugs which affect certain liver enzymes (CYP 3A4 inhibitors including azole antifungals such as ketoconazole, itraconazole). Do not start or stop any medicine without doctor or pharmacist approval.
Adverse/Side Effects
Dry or irritated nose or throat, sneezing, dizziness or headache may occur as your body adjusts to the medication. A bad taste in the mouth may also occur. If any of these effects persist or worsen, inform your doctor. Rinse your mouth after each use if this causes a strange taste in your mouth. Notify your doctor if you develop: rash, sore throat, fever, repeated nasal bleeding, sores in the nose, swelling of the face, wheezing, breathing trouble, behavioral changes (especially in children), green or yellow-colored nasal secretions.
Adverse events involving hypersensitivity reactions and anaphylactic reactions, urticaria, rash, dermatitis, angioedema and pruritus), glaucoma, increased intraocular pressure, cataracts, nasal septum perforation, pharynx disorders (throat irritation, throat pain, swollen throat, burning throat, and itchy throat), anosmia, and palpitations have been reported. Contact your doctor immediately if you experience any of these.
Overdose
Symptoms of Rhinocort overdose are unknown. If you suspect overdose, contact your doctor or local poison control center immediately.
