Ingredients
Active ingredients:
Atacand: candesartan cilexetil
Atacand Plus: candesartan cilexetil, hydrochlorothiazide
Non active: hydroxypropyl cellulose, polyethylene glycol, lactose, corn starch, carboxymethylcellulose calcium, and magnesium stearate. Ferric oxide (reddish brown) is added to the 8-mg, 16-mg, and 32-mg tablets as a colorant

Indications
Atacand is indicated for the treatment of hypertension.

How It Works
Atacand:
Candesartan blocks the vasoconstrictor and aldosterone-secreting effects of angiotensin II by selectively blocking the binding of angiotensin II to the AT1 receptor in many tissues, such as vascular smooth muscle and the adrenal gland. Blockade of the renin-angiotensin system with ACE inhibitors, which inhibit the biosynthesis of angiotensin II from angiotensin I, is widely used in the treatment of hypertension.

Atacand Plus:
Candesartan belongs to a family of medications known as angiotensin II receptor blockers. Hydrochlorothiazide belongs to a family of medication known as diuretics. The combination of these medications is used to lower high blood pressure. Candesartan works by relaxing the blood vessels. This relaxation causes the blood pressure to decrease. Hydrochlorothiazide works by causing the body to rid excess salt and water.

Dosage
Available tablet sizes: 4 mg, 8 mg, 16 mg, 32 mg
Available candesartan/hydrochlorothiazide tablet sizes: 8mg/12.5mg, 16mg/12.5mg)

Hypertension:
The recommended initial dosage and usually maintenance dose is 8 mg once daily. The dose can be increased to 16 mg once daily and can be further increased to 32 mg once daily. If blood pressure control is not achieved with this dose, alternative strategies are recommended.

Use in patients with intravascular volume depletion:
An initial dose of 4 mg may used in patients at risk for hypotension, such as patients with possible volume depletion.

Use in impaired renal function:
The starting dose is 4 mg; the dose should be titrated according to individual response.

Use in impaired hepatic function:
An initial dose of 2 mg once daily is recommended in patients with mild to moderate hepatic impairment. The dose may be adjusted according to response. There is no experience in patients with severe hepatic impairment.

Heart failure:
The usual recommended initial dosage of Atacand is 4 mg daily, then carefully titrated up to a standard dose of 32 mg daily.

Atacand Plus:
The usual adult dose is one tablet to be taken once daily at approximately the same time each day, with or without food.
 
 
Precautions
Consult a doctor before taking Atacand if you are pregnant or breastfeeding, if you are allergic to any of its ingredients, or if you have or have a history of cholestasis, liver disease, kidney disease, heart failure, or hyperkalemia.

As with other agents that inhibit the renin-angiotensin-aldosterone system, changes in renal function may be anticipated in susceptible patients with very severe or end-stage renal impairment. In these patients, Atacand should ne carefully titrated with thorough monitoring of blood pressure. Evaluation of patients with heart failure should include periodic assessments of renal function, especially in elderly patients 75 years or older.

There is a risk of adverse reaction, especially in renal function impairment and hyperkalaemia, that may increase when candesartan is used in combination with an ACE inhibitor. Patients with such treatment should be monitored regularly and carefully. During dialysis the blood pressure may be particularly sensitive to ATI-receptor blockade as a result of reduced.  Patients with such treatment need to be monitored.

Other medicinal products that affect the rennin-angiotensin-aldosterone system, ACE inhibitors, may increase blood urea and serum creatinine in patients with bilateral renal artery stenosis of the artery to a solitary kidney. A similar effect may be anticipated with angiotensin II receptor antagonists.

Hypotension may occur during treatment with Atacand in heart failure patients. Caution should be observed when administrating the medication. Hypotension may occur when used with anesthetic.

In patients whose vascular tone and renal function depend predominantly on the activity of the renin-angiotensin-aldosterone system (e.g. patients with severe congestive heart failure or underlying renal disease, including renal artery stenosis), treatment with other medicinal products that affect this system has been associated with acute hypotension, azotaemia, oliguria or, rarely, acute renal failure. The possibility of similar effects cannot be excluded with angiotensin II receptor antagonists. As with any anti hypertensive agent, excessive blood pressure decrease in patients with ischaemic cardiopathy or ischaemic cerebrovescular disease could result in myocardial infarction or stroke.

Atacand Plus Contraindictaions:  patients with anuria and patients who are hypersensitive to other sulfonamide-derived drugs should speak with a physician.

Drug Interactions
Consult a doctor before taking this medication if you are also taking digoxin, lithium, nonsteroidal anti-inflammatory drugs (NSAIDs), other blood pressure medications, other diuretics, potassium supplements, salt substitutes, contraceptives, nifedipine, enalapril, or glibenclamide.

Atacand Plus:
Consult a doctor if you are also taking alcohol, allopurinol, amantadine, anticholinergic medications (e.g., atropine, biperiden), barbiturates (e.g., secobarbital), calcium supplements, cholestyramine, corticosteroids, cyclophosphamide, cyclosporine, diazoxide, digoxin, diuretics, potassium supplements, lithium, methyldopa, methotrxate, narcotics, norepinephrine, NSAIDs, tubocurarine, vitamin, or warfarin.

Adverse/Side Effects
You may experience dizziness, lightheadedness, blurred vision, sore throat, or a runny/stuffy nose as your body adjusts to the medication. If any of these effects persist or worsen, notify your doctor or pharmacist promptly.

Tell your doctor immediately if any of these unlikely but serious side effects occur: fainting, decreased sexual ability, change in the amount of urine, signs of infection (e.g., fever, chills, persistent sore throat), stomach/abdominal pain, severe nausea, yellowing eyes or skin, dark urine, unusual fatigue, chest pain.

An allergic reaction to this drug is unlikely, but seek immediate medical attention if it occurs. Symptoms of an allergic reaction include: rash, itching, swelling (especially of the face, lips, tongue, or throat), severe dizziness, trouble breathing. If you notice other effects not listed above, contact your doctor or pharmacist.

Atacand Plus:
Common side effects include back pain, arthralgia, fatigue, abdominal pain, urinary tract infection, nausea, diarrhea, gastrocentritis, tachycardia, ECG abnormality, peripheral edema, chest pain, hyperuricemia, hyperglycemia, headache, dizziness, nervous disorder causing self injury, upper respiratory, influenza-like symptoms

Less common incidents that need to be reported to a doctor: syncope, angina pectoris, circulatory failure, flushing, hypotension, myocardial infarction, peripheral ischemia, thrombophebitis, hypertonia, hypoesthesia, paresthesia, vertigo, constipation, dyspepsia, dry mouth, toothache, tinnitus, diabetes mellitus, hyperkalemia, hyponatramia, arthritis, arthropathy, myalgia, myopathy, skeletal pain, tendon disorder, anemia, epistaxis, depression, impotence, neurosis, menopausal symptoms, otitis, laryngitis, pruritus, rash, skin disorder, sweating, urticaria, abnormal urine, cystitis, conjunctivitis, etc.

Overdose
Symptoms of overdose include unusually fast or slow heartbeat, severe dizziness or fainting. If overdose is suspected, contact your local poison control center or emergency room immediately.

Storage Instructions
Store at room temperature (77 degrees F or 25 degrees C), away from light and moisture. Brief storage between 59 and 86 degrees F (15 to 30 degrees C) is permitted. Do not store in the bathroom.
