Indications
Capoten is indicated in the treatment of high blood pressure, heart failure, left ventricular dysfunction after myocardial infarction, and diabetic nephropathy.

How It Works
Captopril is an angiotensin-converting enzyme (ACE) inhibitor. Angiotensin is a protein that has the effect, when released by the human body, of narrowing blood vessels. Angiotensin is a normal part of the body, but when blood pressure reaches dangerous levels, or when the blood vessels must be prevented from narrowing, its function may inadvertently become pathological or life-threatening. ACE inhibitors inhibit the action of angiotensin, thereby preventing the blood vessels from narrowing to the point that they endanger people with hypertension or other cardiovascular diseases.

Dosage
Available tablet sizes: 25 mg

Capoten dosages should be set after careful consultation with a health care professional, and carefully titrated to your personal requirements. All Capoten doses should be taken one hour before meals, and proper fluid intake should be maintained to prevent dehydration. In case of renal impairment, smaller or less frequent doses may suffice.

Hypertension:
The initial dosage is 25 mg taken two or three times daily. If a sufficient reduction in blood pressure has not been achieved after two weeks, the dose may be increased to 50 mg twice or thrice daily. If the blood pressure has still not been adequately controlled after two weeks at 50 mg, a diuretic may be added to your regimen (provided you are not already taking one). If Capoten is being started in a patient already taking a diuretic, close medical supervision should be initiated.

After this point, if further blood pressure reduction is still required, Capoten dosage may be increased to 100 mg two or three times daily, and then (in special cases) up to 150 mg twice or thrice daily. The maximum dose is 450 mg daily – do not exceed this.

Heart failure:
Capoten should generally be used in conjunction with a diuretic and digitalis in the treatment of heart failure. It should also be used under careful medical supervision. Initiation of therapy requires consideration of recent diuretic therapy and the possibility of severe salt/volume depletion. In patients with either normal or low blood pressure, who have been vigorously treated with diuretics and who may be hyponatremic and/or hypovolemic, a starting dose of 6.25 or 12.5 mg thrice daily may minimize the magnitude or duration of the hypotensive effect; for these patients, titration to the usual daily dosage can then occur within the next several days.

For most patients the usual initial daily dosage is 25 mg thrice daily. After a dose of 50 mg thrice daily is reached, further increases in dosage should be delayed, where possible, for at least two weeks to determine if a satisfactory response occurs. Most patients studied have had a satisfactory clinical improvement at 50 or 100 mg thrice daily. A maximum daily dose of 450 mg should not be exceeded.

Left ventricular dysfunction after myocardial infarction:
The recommended dose for long-term use in patients following a myocardial infarction is a target maintenance dose of 50 mg thrice daily. Therapy may be initiated as early as three days following a myocardial infarction. After a single dose of 6.25 mg, Capoten therapy should be initiated at 12.5 mg thrice daily. Dosage should then be increased to 25 mg thrice daily during the next several days and to a target dose of 50 mg thrice daily over the next several weeks as tolerated

Diabetic nephropathy:
The recommended dosage for long term treatment of diabetic nephropathy is 25 mg three times daily.

Precautions
Consult a doctor before taking Capoten if you are pregnant or breastfeeding, if you are allergic to any of its ingredients, or if you have or have a history of any allergies, kidney disease, liver disease, high blood levels of potassium, heart problems, severe dehydration, diabetes, strokes, blood vessel disease, lupus or scleroderma.

Be careful not to become too overheated during exercise which can lead to excessive sweating. Consult your doctor if you experience severe vomiting or diarrhea.

This medication may make you more sensitive to the sun. Avoid prolonged sun exposure, tanning booths or sunlamps. Use a sunscreen and wear protective clothing when outdoors.

Before having surgery, tell your doctor or dentist that you are taking this medication.

Caution is advised when using this drug in children and the elderly because they may be more sensitive to its effects, especially dizziness

Drug Interactions
Consult a doctor before taking Capoten if you are taking immunosuppressants, other heart medications, other high blood pressure medications, lithium, non-steroidal anti-inflammatory drugs (NSAIDs), diuretics, potassium supplements, salt substitutes, trimethoprim, or any drugs that may increase your heart rate or blood pressure.

Adverse/Side Effects
You may experience dizziness, light-headedness, loss of taste, dry cough or blurred vision as your body adjusts to the medication. If any of these effects persist or worsen, notify your doctor or pharmacist promptly.

Tell your doctor immediately if any of these unlikely but serious side effects occur: fainting, decreased sexual ability, fast/pounding heartbeat, chest pain, a change in the amount of urine, signs of infection (e.g., fever, chills, persistent sore throat).

This drug may rarely cause serious (possibly fatal) liver problems. If you notice any of the following highly unlikely but very serious side effects, seek immediate medical attention: yellowing eyes or skin, dark urine, stomach/abdominal pain, persistent fatigue, persistent nausea.

A serious allergic reaction to this drug is unlikely, but seek immediate medical attention if it occurs. Symptoms of a serious allergic reaction include: rash, itching, swelling (especially of the face, lips, tongue, or throat), severe dizziness, trouble breathing.

Overdose
Symptoms of overdose include unusually fast or slow heartbeat, severe dizziness, or fainting. If you suspect overdose, contact your local poison control center or emergency room immediately.

Storage Instructions
Store at room temperature, away from moisture and light, and out of reach of children and pets.
