Indications
Cardizem XL is indicated in the treatment of high blood pressure (hypertension), irregular heartbeats, and chest pain (angina).

How It Works
Diltiazem is a calcium channel blocker. Calcium is involved in blood vessel contraction and in controlling the electrical impulses within the heart. By blocking calcium, Diltiazem relaxes and widens blood vessels and can normalize the heart’s rhythm. Diltiazem is used to treat chest pain (angina) or high blood pressure. Diltiazem relaxes blood vessels so the heart does not have to pump as hard. It also increases the supply of blood and oxygen to the heart.
 
Dosage
Tablets (30 mg, 60 mg, 90 mg, 120 mg, 180 mg extended release)

Take this medication orally once daily or as directed by your doctor. Your doctor will probably start you on a low dose of Diltiazem and gradually increase your dose. Do not increase your dose more than once every 7 to 14 days.

Try to take it at the same time each day. It is important to continue taking this medication even if you feel well. Most people with high blood pressure do not feel sick. Do not stop taking this medication without consulting your doctor. Some conditions may become worse when the drug is abruptly stopped. Your dose may need to be gradually decreased.

Precautions
You should not use this medication if you are allergic to Diltiazem or any of the other ingredients in it. Diltiazem lowers blood pressure and may cause dizziness or fainting, especially when you first start taking it. Be careful when standing up or sitting up from a lying position. If you stopped taking Diltiazem for any reason and have started taking it again, you may feel especially dizzy or light-headed after your first few doses.

Tell your doctor your whole medical history before taking this drug, especially: heart disease, liver disease, lung disease, or any allergies. If you’re having surgery during your treatment with Diltiazem, tell the surgeon ahead of time. Do not stop using the medication before surgery unless your surgeon tells you to.

It is unknown whether this medication can harm an unborn baby. Before taking Diltiazem, tell your doctor if you are pregnant or plan to become pregnant during treatment. Diltiazem does pass into breast milk and may cause harm to a nursing baby. Consult your doctor before using this medication if you are breast-feeding a baby.

This drug may make you dizzy or drowsy. You should not drive, use machinery, or do any activity that requires alertness until you are sure you can perform such activities safely. Do not drink excessively as alcohol drowsiness may i


Drug Interactions
Do not start, stop or change the dosage of any medicine before checking with your doctor first. Before using this medication, tell your doctor or pharmacist about all prescription and nonprescription medications, including herbal products you may use, especially: Cyclosporine, Flecainide, beta-blockers (including eye drops), Digoxin, Lithium, Disopyramide, high blood pressure medication, Quinidine, Rifampin, Carbamazepine, cimetidine, Atazanavir ,Reyataz, Benzodiazepines such as midazolam, Versed and Triazolam, Halcion; beta blockers such as Atenolol (Tenormin), Labetalol, Trandate, Metoprolol, Lopressor, Toprol XL, Nadolol, Corgard, and Propranolol, Inderal; Buspirone, BuSpar, Carbamazepine, Carbatrol, Epitol, Tegretol, Cimetidine, Tagamet,  Cyclosporine, Gengraf, Neoral, Sandimmune, Digoxin, Lanoxin, Lanoxicaps,  Lovastatin, Altoprev, Mevacor, Advicor, Quinidine, Quinidex, Rifampin, Rifadin,  Rifamate, Rifater and Rimactane. Many other medications may also interact with Diltiazem, so be sure to tell your doctor about all the medications you are taking, even those that do not appear on this list. Avoid any drugs that increase your heart-rate (decongestants Phenylephrine, Pseudoephedrine and Phenyipropanolamine for example). Be aware that some of these drugs are commonly found in over-the-counter cough-and-cold products.

Consult a doctor before taking Cardizem XL if you are taking any other blood pressure  medications or drugs for sexual function problems (sildenafil, tadalafil, vardenafil). Do not start or stop any medicine without doctor or pharmacist approval.

Check the labels on all your medicines (cough-and-cold products, diet aids) because they may contain ingredients that could increase your heart rate or blood pressure. Ask your pharmacist about using those products safely.

Adverse/Side Effects
Dizziness, light headedness, nausea, flushing, drowsiness, trouble sleeping, joint/muscle pain, headache, weakness, slow heartbeat, vomiting, diarrhea, constipation, nasal congestion, or cough. If any of these effects persist or worsen, tell your doctor or pharmacist promptly. Remember that your doctor has prescribed this medication because he or she has judged that the benefit to you is greater than the risk of side effects. Many people using this medication do not have serious side effects.

Some side effects can be much more serious and can indicate other problems, immediately contact your doctor if you experience any of the following side effects: Swelling of the face, eyes, lips, tongue, hands, arms, feet, ankles, or lower legs, difficulty breathing or swallowing, fainting, rash, yellowing of the skin or eyes, nausea, extreme tiredness, unusual bleeding or bruising, lack of energy, loss of appetite, pain in the upper right part of the stomach, flu-like symptoms, increase in frequency or severity of chest pain (angina).

A very serious allergic reaction to this drug is rare. Get emergency medical help if you have any of these signs of an allergic reaction: Rash, itching/swelling (especially of the face/tongue/throat), severe dizziness, trouble breathing.

Overdose
Symptoms of overdose include dizziness, weakness, slowed or irregular heartbeat, nausea, drowsiness, confusion, and slurred speech. If you suspect overdose, contact your local poison control center or emergency room immediately.

Store at room temperature, away from moisture and light, and out of reach of children and pets.
