Ingredients
Active ingredients: benazepril
Non-active ingredients: colloidal silicon dioxide, crospovidone, hydrogenated castor oil (5-mg, 10-mg, and 20-mg tablets), hypromellose, iron oxides, lactose, magnesium stearate (40-mg tablets), microcrystalline cellulose, polysorbate 80, propylene glycol (5-mg and 40-mg tablets), starch, talc, and titanium dioxide.

Indications
Lotensin is indicated in the treatment of high blood pressure (hypertension).

How It Works
Lotensin works by relaxing blood vessels, causing them to widen. This alleviates the pressure they exert on the blood. Lowering high blood pressure helps prevent strokes, heart attacks and kidney problems.

Dosage
Available oral tablet sizes: 5 mg, 10 mg
 
The recommended initial dose for patients not receiving a diuretic is 10 mg once a day. The usual maintenance dosage range is 20-40 mg per day administered as a single dose or in two equally divided doses. A dose of 80 mg can give an increased response, but experience with dosages this high is limited and not recommended.

Hypertensive Patients with Renal Impairment:
For patients with a creatinine clearance < 30 mL/min/1.73 m2 (serum creatinine > 3 mg/dL), the recommended initial dose is 5 mg Lotensin once daily.

Children:
In children, doses of Lotensin between 0.1 and 0.6 mg/kg once daily have been studied, and doses greater than 0.1 mg/kg were shown to reduce blood pressure (see Pharmacodynamics). Based on this, the recommended starting dose of Lotensin is 0.2 mg/kg once per day as monotherapy. Doses above 0.6 mg/kg (or in excess of 40 mg daily) have not been studied in pediatric patients.
 
 
Precautions
Before you begin using a medication, be sure to inform your doctor of any medical conditions or allergies you may have, any medications you are taking, whether you are pregnant or breast-feeding, and any other significant facts about your health.

Fluid and electrolyte balance:
Increases in blood levels of potassium occur in approximately 1.1% of patients receiving benazepril. This rarely causes problems, but potassium levels should be monitored by your doctor.

Kidney function:
Changes in kidney function have been seen in certain people who use benazepril (e.g., people with narrowed blood vessels in their kidneys, or those with severe congestive heart failure). The use of diuretics (water pills) or nonsteroidal anti-inflammatory drugs (NSAIDs) may further increase the risk of kidney problems for those people already at risk.

Low blood pressure:
Occasionally, blood pressure drops too low after taking benazepril. This usually happens after the first or second dose or when the dose is increased. It is more likely to occur in those who take water pills, have a salt-restricted diet, are on dialysis, are suffering from diarrhea or vomiting, or have been sweating excessively and not drinking enough liquids. If low blood pressure causes you to faint or feel lightheaded, contact a doctor.

Children:
The safety and effectiveness of benazepril for use by children have not been established. Its use by this age group is not recommended.

Drug Interactions
There may be an interaction between benazepril and any of the following: diuretics (water pills), lithium, medications that increase potassium levels (e.g., potassium supplements, salt substitutes containing potassium), other medications that lower blood pressure, and nonsteroidal anti-inflammatory medications (NSAIDs; e.g., ibuprofen, indomethacin, naproxen).

Adverse/Side Effects
Angioedema (a serious allergic reaction which causes the area around the throat and tongue to swell) may occur with ACE inhibitors, including benazepril. If you experience swelling of the face, tongue, or throat, stop taking benazepril at once and get immediate medical attention. Other medications known as ACE inhibitors should not be taken in the future. People who have had angioedema caused by other substances may be at increased risk of angioedema while receiving an ACE inhibitor.

Common side effects include a dry, persistent cough, headache, diarrhea, loss of taste, nausea, and unusual tiredness. If these effects persist or worsen, contact your doctor.

Contact your doctor immediately if any of these more serious side effects manifest: signs of low blood pressure (dizziness, lightheadedness, fainting), skin rash with or without itching, fever, joint pain, abdominal pain, abdominal distention, fever, nausea, or vomiting, chest pain, signs of too much potassium in the body, confusion, irregular heartbeat, nervousness, numbness or tingling in hands, feet, or lips, shortness of breath or difficulty breathing, and weakness or heaviness of legs.

Stop taking the medication and seek immediate medical attention if any of the following occur: fever and chills, hoarseness, stomach pain, itching of skin, or yellow eyes or skin, swelling of face, mouth, hands, or feet (signs of angioedema), trouble swallowing or breathing (sudden).

Overdose
Symptoms of overdose may include: unusually fast or slow heartbeat, severe dizziness, or fainting. If you suspect overdose, please contact your local emergency and poison control

Storage Instructions
Lotensin should be stored 59- 86°F (15-30°C) and be protected from excessive light and humidity. It should be kept in a tight, light-resistant container.
