Chemical Names
Amlodipine/benazepril

Ingredients
Active ingredients: amlodipine, benazepril
Non-active ingredients: calcium phosphate, cellulose compounds, colloidal silicon dioxide, crospovidone, gelatin, hydrogenated castor oil (not present in 5/40 mg or 10/40 mg strengths), iron oxides, lactose, magnesium stearate, polysorbate 80, silicon dioxide, sodium lauryl sulfate, sodium starch (potato) glycolate, starch (corn), talc, and titanium dioxide.

Indications
Lotrel is a combination of two drugs, a calcium channel blocker (amlodipine) and an ACE inhibitor (benazepril). It is used to treat high blood pressure (hypertension).

How It Works
Lotrel works by relaxing blood vessels, causing them to widen. Lowering high blood pressure helps prevent strokes, heart attacks and kidney problems.

Dosage
Available oral tablet sizes (amlodipine/benazepril): 5mg/10mg
 
Amlodipine is an effective treatment of hypertension in once-daily doses of 2.5-10 mg while benazepril is effective in doses of 10-80 mg. The exact amount you should take is dependent on your response to each individual medication and a doctor’s recommendation.

Renal Impairment
Regimens of therapy with Lotrel need not take account of renal function as long as the patient's creatinine clearance is > 30 mL/min/1.73m² (serum creatinine roughly ≤ 3 mg/dL or 265 μmol/L).

Hepatic Impairment and Elderly Patients
The recommended initial dose of amlodipine, as monotherapy or as a component of combination therapy, is 2.5 mg.

Precautions
Consult a doctor before taking Lotrel if you are pregnant or breastfeeding, if you are allergic to any of its ingredients, or if you have or have a history of allergies, kidney disease, liver disease, high blood levels of potassium, heart problems (e.g., congestive heart failure, coronary artery disease), severe dehydration (and loss of electrolytes such as sodium), diabetes (poorly controlled), or blood vessel disease (e.g., collagen vascular diseases such as lupus, scleroderma).

This drug may make you dizzy; use caution engaging in activities requiring alertness such as driving or using machinery. Limit alcoholic beverages. To minimize dizziness and light-headedness due to lowering of your blood pressure, get up slowly when rising from a seated or lying position.

Serious loss of body fluids can also lower your blood pressure and worsen dizziness. Drink adequate fluids to prevent from becoming dehydrated. If you are on restricted fluid intake, consult your doctor for further instructions. Be careful not to become too overheated during exercise which can lead to excessive sweating.

This drug can cause serious fetal harm (possibly death) if used during the last 6 months of pregnancy. If you become pregnant or think you may be pregnant, contact your doctor immediately.

Drug Interactions
Before using this medication, tell your doctor or pharmacist of all prescription and nonprescription products you may use, especially: immunosuppressants, other heart drugs (e.g., digoxin), other high blood pressure medications, intravenous (IV) calcium, lithium, non-steroidal anti-inflammatory drugs (NSAIDs such as celecoxib, ibuprofen, indomethacin), potassium supplements (e.g., potassium chloride) or salt substitutes, trimethoprim-containing medications (e.g., sulfamethoxazole/trimethoprim), or diuretics.

Check the labels on all your medicines (e.g., cough-and-cold products, diet aids) because they may contain ingredients that could increase your heart rate or blood pressure. Ask your pharmacist about the safe use of those products. Do not start or stop taking Lotrel without doctor or pharmacist approval.

Adverse/Side Effects
You may experience dizziness, light-headedness, fatigue, or dry cough as your body adjusts to the medication. If any of these effects persist or worsen, notify your doctor or pharmacist promptly. Tell your doctor immediately if any of these unlikely but serious side effects occur: fainting, decreased sexual ability or interest. Tell your doctor immediately if any of these highly unlikely but very serious side effects occur: chest pain, change in the amount of urine, signs of infection (e.g., fever, chills, persistent sore throat). This drug may rarely cause serious (possibly fatal) liver problems. If you notice any of the following highly unlikely but very serious side effects, seek immediate medical attention: yellowing eyes or skin, dark urine, stomach/abdominal pain, persistent fatigue, persistent nausea.

A serious allergic reaction to this drug is unlikely, but seek immediate medical attention if it occurs. Symptoms of a serious allergic reaction include: rash, itching, swelling (especially of the face, lips, tongue, or throat), severe dizziness, trouble breathing. If you notice other effects not listed above, contact your doctor or pharmacist.

Overdose
Symptoms of overdose include dizziness, fainting, and unusually fast or slow heartbeat. If you suspect overdose, contact your local poison control center or emergency room immediately.
