Moduretic, amiloride and hydrochlorothiazide

Chemical Names
amiloride + hydrochlorothiazide

Ingredients
Active Ingredients: amiloride + hydrochlorothiazide
Non-Active Ingredients: calcium phosphate, FD&C Yellow 6, guar gum, lactose, magnesium stearate and starch.

Indications
Moduretic is indicated in the treatment of high blood pressure (hypertension) and congestive heart failure, especially in patients who develop hypokalemia when thiazides or other kaliuretic diuretics are used alone, or in whom maintenance of normal serum potassium levels is considered to be clinically important, e.g., digitalized patients, or patients with significant cardiac arrhythmias.

How It Works
Moduretic is a combination of amiloride, a potassium-sparing diuretic, and hydrochlorothiazide, a thiazide diuretic. Hydrochlorothiazide inhibits the kidneys’ ability to retain water, while amiloride works to trigger the release of salt and water from the bloodstream while sparing potassium. Taken together, these drugs work to lower blood pressure by decreasing the volume of water in the blood.

Dosage
Available tablet sizes (amiloride/hydrochlorothiazide): 5mg/50mg

The standard starting dosage of Moduretic is 1 tablet a day, taken with food to prevent nausea. If necessary, the dosage may be increased to 2 tablets a day, but no more than this. Moduretic is usually taken as a single dose, but this may be divided at a doctor’s recommendation.

Do not stop taking this drug without a doctor’s consent.

Precautions
Consult a doctor before taking Moduretic if you are pregnant or breastfeeding, if you are allergic to any of its ingredients, or if you have a history of diabetes, kidney disease (especially metabolic acidosis), liver disease, asthma, or any drug allergies.

Like other potassium-sparing diuretics, Moduretic may cause hyperkalemia (elevated serum potassium levels), especially in patients with impaired kidney function.  When left uncorrected, hyperkalemia can be fatal, thus serum electrolyte levels should be carefully monitored while taking Moduretic.

Do not consume alcohol or narcotics while taking Midamor.

This fixed combination drug is not indicated for the initial therapy of edema or hypertension except in individuals in whom the development of hypokalemia cannot be risked.

Drug Interactions
Consult a doctor before taking Moduretic if you are taking nonsteroidal anti-inflammatory drugs (NSAIDs), angiotensin-converting enzyme (ACE) inhibitors (i.e.: captopril, lisinopril), angiotensin receptor blockers (ARBs such as losartan and valsartan), digoxin, lithium, potassium supplements, triamterene, spironolactone, medication for arthritis, diabetes medications, barbiturates, cholestyramine, colestipol resins, corticosteroids, norepinephrine, muscle relaxants, thyroid medication, or any other high blood pressure drugs.

Adverse/Side Effects
Moduretic is generally well tolerated. While rare, the most common known side effects include: nausea/anorexia, abdominal pain, flatulence, and mild skin rash. If these effects persist or worsen, contact your doctor.

Other side effects include headache, weakness, fatigue, arrhythmia, diarrhea, gastrointestinal pain, nervousness, dsypnea, and pruritus.
