Monopril, fosinoprilChemical NameFosinopril Ingredients Active Ingredients: Fosinopril Non-Active Ingredients: Lactose, microcrystalline cellulose, crospovidone, povidone, and sodium stearyl fumarate. Indications Monopril is used to treat hypertension, in the management of heart failure as adjunctive therapy. How It Works Monopril is in a group of drugs called ACE inhibitors. ACE stands for angiotensin converting enzyme. Monopril is used to treat high blood pressure (hypertension) or heart failure. Dosage Tablets (10 mg, 20 mg) Hypertension: The recommended initial dose of Monopril (fosinopril sodium tablets) is 10 mg once a day, both as mono-therapy and when the drug is added to a diuretic. Dosage should then be adjusted according to blood pressure response at peak (2-6 hours) and through (about 24 hours after dosing) blood levels. The usual dosage range needed to maintain a response at through is 20-40 mg but some patients appear to have a further response at 80 mg. Heart Condition: The usual starting dose of Monopril should be 10 mg once daily. Following the initial dose of Monopril, patient should be observed under medical supervision for at least 2 hours for the presence of hypotension or orthostasis, and if necessary, until blood pressure stabilizes. An initial dose of 5 mg is also occasionally given. Dosage should be increased, over a several week period, to a dose that is maximal and tolerated but not exceeding 40 mg once daily. Monitoring of patients is warranted. If this happens discontinue use and commence appropriate therapy. This medication may cause swelling of the walls of the bowel. This will result in abdominal pain. If pain becomes bothersome contact a physician. Patients receiving treatment with hymenoptera venom or patients dialyzed with high-flux membranes while receiving ACE inhibitors sustained life-threatening anaphylactic reactions. Monitoring is necessary during concomitant treatment with other drugs. Monopril can cause hypotension. In patients with kidney failure may cause excessive hypotension. If hypotension occurs, treat with intravenous infusion of physiological saline. In rare instances Monopril has resulted in jaundice caused by the obstruction of bile and death of liver tissue. There has been increase in blood urea nitrogen and serum creatinine, usually minor and transient, especially when Monopril has been given concomitantly with a diuretic. There has been serum potassium greater than 10% above the upper limit of normal when taking this medication. Risk factors for the development of hyperkalemia include renal insufficiency, diabetes mellitus, and the concomitant use of potassium-sparing diuretics, potassium supplements, and/or potassium-containing salt substitutes, which should be used cautiously. Monopril can cause impaired renal function. Anesthesia used during surgery can cause hypotension when used with Monopril. This medicine may cause harm to an unborn baby. Before taking Monopril, tell your doctor if you are pregnant or plan to become pregnant during treatment. Monopril may pass into breast milk and may causes harm to a nursing baby. Consult your doctor before using this medication if you are pregnant or breast-feeding a baby. Rare side effects: fever, influenza, weight gain, hyperhidrosis, sensation of cold, fall, pain, sudden death, heart attack, respiratory attack, shock, atrial rhythm disturbance, angioedema, cardiac rhythm disturbances, non-anginal chest pain, edema lower extremity, hypertension, syncope, conduction disorder, bradycardia, tachycardia, sinus abnormality, pruritus, gout, numbness, sexual dysfunction, hepatomegaly, abdominal distention, decreased appetite, depression, dry mouth, constipation, flatulence, muscle ache, swelling of an extremity, weakness of an extremity, cerebral infarction, stroke, paresthesia, vertigo, behavior change, tremor, abnormal vocalization, rhinitis, tracheobronchitis, abnormal breathing, pleuritic chest pain, yellowing of skin and eyes. Special Senses: Vision disturbance, taste disturbance. Urogenital: Abnormal urination, kidney pain. If any of these side effects become bothersome seek medical attention immediately. A very serious allergic reaction to this drug is rare. Get emergency medical help if you have any of these signs of an allergic reaction: rash, itching/swelling (especially of the face/tongue/throat), severe dizziness, or trouble breathing.
