Ingredients
Active Ingredients: bisoprolol, hydrochlorothiazide
Non-Active Ingredients: colloidal silicon dioxide, corn starch, dibasic calcium phosphate, hypromellose, magnesium stearate, microcrystalline cellulose, polyethylene glycol, polysorbate 80, and titanium dioxide. The 5 mg/6.25 mg tablet also contains Red and Yellow Iron Oxide. The 2.5 mg/6.25 mg tablet also contains crospovidone, pregelatinized starch, and yellow iron oxide.

Indications
Ziac is indicated in the treatment and management of high blood pressure (hypertension).

How It Works
Ziac combines bisoprolol, a beta-blocker, with hydrochlorothiazide, a diuretic. Beta-blockers inhibit the excitatory action of adrenaline, epinephrine and norepinephrine on the heart, thereby reducing its contractive force, which lowers blood pressure. Diuretics encourage the removal of water from the bloodstream, which reduces blood volume and thus blood pressure.

Dosage
Available tablet sizes (bisoprolol/hydrochlorothiazide): 2.5mg/6.25mg, 5mg/6.25mg

Bisoprolol is effective in treating hypertension in single daily doses from 2.5 mg to 40 mg, while hydrochlorothiazide’s effective range in single daily doses is from 12.5 mg to 50 mg. Treatment begins with the lowest possible dosage – one 2.5mg/6.25mg tablet daily. Dosage is then increased carefully (at 14 day intervals), under close medical supervision, until an effective maintenance dosage is reached.

Cessation of Ziac therapy should take place gradually over a period of 2 weeks.

Elderly patients should use a smaller amount of Ziac if possible, due to decreased clearance rates of the drug.
 

Precautions
Consult a doctor before taking Ziac if you are pregnant or breastfeeding, if you are allergic to any of its ingredients, or if you have or have a history of heart failure, bronchospastic disease, peripheral vascular disease, thyroid problems, kidney disease, liver disease, potassium deficiency, gout, or hyperuricemia.

Drug Interactions
Consult a doctor before taking Ziac if you are taking other high blood pressure medications, clonidine, heart medications (especially antiarrhythmics such as disopyramide), calcium antagonists (such as verapamil and diltiazem), rifampin, alcohol, barbiturates, narcotics, antidiabetics, cholestyramine, colestipol resins, corticosteroids, norepinephrine, muscle relaxants, lithium, and nonsteroidal anti-inflammatory drugs (NSAIDs).

Adverse/Side Effects
Common bisoprolol side effects include abdominal cramps, diarrhea, dizziness, fatigue, depression, headache, nausea, impotence, slow heart rate, hypotension, numbness, tingling, cold extremities, sore throat, and difficulty breathing. If any of these side effects continue or worsen, contact your doctor immediately.

Common hydrochlorothiazide side effects include muscle weakness, dizziness, cramps, thirst, stomach pain, upset stomach, vomiting, diarrhea, loss of appetite, headache, and hair loss. If any of these side effects continue or worsen, contact your doctor immediately.

More serious hydrochlorothiazide side effects include sore throat with fever, unusual bleeding or bruising, severe skin rash with peeling skin, and difficulty breathing and swallowing. If you experience any of these effects, contact your doctor immediately.

Overdose
Symptoms of Ziac overdose include bradycardia, hypotension, lethargy, sedation, bronchospasm, heart failure, dehydration, and loss of electrolytes. If you suspect overdose, contact your local poison control center or emergency room immediately.

Storage Instructions
Store at room temperature, away from moisture and light, and out of reach of children and pets.
