Indications
Leukeranis indicated in the treatment of chronic lymphatic (lymphocytic) leukemia, malignant lymphomas including lymphosarcoma, giant follicular lymphoma, and Hodgkin's disease, Cancer of the ovaries, cancer of the lymph system that affects the skin, hairy cell leukemia (a cancer of the blood and bone marrow), nephrotic syndrome (a kidney disease), tumors in the uterus (womb), Waldenström's macroglobulinemia (a certain type of cancer of the blood), Histiocytosis X (a certain type of cancer found primarily in children)
 
How It Works
Leukeran is an antineoplastic in the class of alkylating agents and is used to treat various forms of cancer. Antineoplastics stop tumor growth by cross-linking guanine bases in DNA double-helix strands directly attacking DNA. This makes the strands unable to uncoil and separate, preventing DNA replication. They also cause miscoding of DNA, disrupting DNA function and causing cell death.
 
Dosage
Tablet (2 mg)
 
Initial dosage: The usual oral dosage is 0.1 to 0.2 mg/kg body weight daily for 3 to 6 weeks as required. This usually amounts to 4 to 10 mg per day for the average patient. The entire daily dose may be given at one time.
 
The dosage must be carefully adjusted according to the response of the patient and must be reduced as soon as there is an abrupt fall in white blood cell count.
 
Hodgkin’s disease usually requires 0.2 mg/kg daily.
 
Lyphoma or chronic lymphocytic leukemia: 0.1 mg/kg daily. Patients with Lymphocytic infiltration of the bone marrow should not exceed 0.1 mg/kg.
 
Maintenance dosage: If maintenance dosage is used, it should not exceed 0.1 mg/kg daily and may well be as low as 0.03 mg/kg daily. A typical maintenance dose is 2 mg to 4 mg daily, or less, depending on the status of the blood counts.
 
 
Precautions
Chlorambucil should not be used in patients whose disease has demonstrated a prior resistance to the agent. Patients who have demonstrated hypersensitivity to chlorambucil should not be given the drug. There may be cross-hypersensitivity (skin rash) between chlorambucil and otheralkylating agents.
 
The presence of other medical problems may affect the use of this medicine. Make sure you tell your doctor if you have any other medical problems, especially: Chickenpox (including recent exposure) or Herpes zoster (shingles), convulsions (seizures), past head injury, gout or kidney stones, Chlorambucil may increase levels of uric acid in the body, which can cause gout or kidney stones. Convulsions, infertility, leukemia, and secondary malignancies have been observed when chlorambucil was employed in the therapy of malignant and non-malignant diseases.
 
This medication can lower your body's ability to fight an infection. If you develop a fever, chills or sore throat contact your doctor immediately.
 
Do not have immunizations/vaccinations without consent of your doctor, and avoid contact with people who have recently received oral polio vaccine.
 
Leukeran is not recommended for use during pregnancy. If you are pregnant or thinking of becoming pregnant while on Leukeran speak to your doctor. Chlorambucil can cause fetal harm when administered to a pregnant woman.
 
Chlorambucil has been shown to cause chromatid or chromosome damage in humans. Many patients develop a slowly progressivelymphopenia during treatment.
 
A high incidence of sterility has been documented when chlorambucil is administered to prepubertal and pubertal males.
 
Chlorambucil should not be given at full dosages before 4 weeks after a full course of radiation therapy or chemotherapy because of the vulnerability of the bone marrow to damage under these conditions
 
While it is not necessary to stop Leukeran if there is a fall in neutrophil count, it must be remembered that the fall may continue for 10 days after the last dose, and that as the total dose approaches 6.5 mg/kg, there is a risk of causing irreversible bone marrow damage.
 
Children with nephrotic syndrome and patients receiving high pulse doses of chlorambucil may have an increased risk of seizures.
 
Drug Interactions
Using this medicine with other medicines is usually not recommended, but may be required in some cases. If several medicines are prescribed together, your doctor may change the dose or how often you use one or both of the medicines.
Talk to your doctor if you are taking any of the following live vaccines (e.g., mumps, measles, yellow fever), natalizumab, phenylbutazone or if you are getting any new vaccines or shots. These can be very hazardous as the immune system is already in a weakened state.
 
Tell your doctor or pharmacist about all prescription, over-the-counter (non-prescription), supplements and herbal medications you may be taking. 
 
Adverse/Side Effects
If you experience any infrequent side effects tell your doctor immediately if they are bother some and should they persist, common side effects:
 
Bone marrow suppression, anemia, low white blood cell count: A low platelet count and a shortage of all types of blood cells, nausea, vomiting, diarrhea, oral ulceration.
 
If you experience any of these rare side effects immediately contact your doctor: fever, chills, painful or difficult urination, easy bruising or bleeding, black, tarry stools, blood in urine or stools, small red spots on the skin, joint pain, swollen feet or lower legs, sores in mouth or on lips, yellow eyes or skin, cough, lower back or side pain, stomach pain, trouble walking, muscle twitching, agitation, confusion, hallucinations, weakness, paralysis, tremors, myoclonia, confusion, agitation, ataxia, flaccid paresis, hallucinations, local and/or generalized seizures.
 
Allergic reactions are rare but if you think you are experiencing an allergic reaction call a doctor immediately. Allergic reaction symptoms include: urticaria and angioneurotic edema, shin rash, toxic epidermal necrolysis and body wide allergic reaction involving the skin andmucousmembranes.
 
Overdose
In case you experience and overdose call you local emergency room or poison control center immediately. An overdose can be fatal, if you suspect you've taken too much call a doctor.
 
Storage Instructions
Store in the refrigerator between (2 to 8 degrees C) 36 to 46 degrees F.
