How It Works Caduet combines amlodipine, a calcium channel blocker, with atorvastatin, a cholesterol-lowering medication. Calcium channel blockers decrease the force of contraction of the heart, lowering its oxygen demands and reducing hypertension. Statins inhibit the enzyme responsible for cholesterol synthesis in the liver, lowering blood cholesterol levels; they also increase the number of low density lipoprotein (LDL) receptors, lowering blood lipid levels. Dosage Available tablet sizes (amlodipine/atorvastatin): 5mg/10mg The usual dosage of Caduet is one to two tablets taken once daily, depending on the severity of the condition and the patient’s response. Hypercholesterolimia is usually treated with Caduet in conjunction with another antihypercholesterolimic. What follows is more specific information on the indications of amlodipine and atorvastatin in the treatment of their approved conditions: Hypertension: The usual initial antihypertensive oral dose of amlodipine is 5 mg once daily with a maximum dose of 10 mg once daily. Small, fragile, or elderly individuals, or patients with hepatic insufficiency may be started on 2.5 mg once daily and this dose may be used when adding amlodipine to other antihypertensive therapy. Dosage should be adjusted according to each patient’s need. Angina: The recommended dose of amlodipine for chronic stable or vasospastic angina is 5-10 mg, with the lower dose suggested in the elderly and in patients with hepatic insufficiency. Most patients will require 10 mg for adequate effect. Coronary artery disease: The recommended dose range of amlodipine for patients with coronary artery disease is 5-10 mg once daily. In clinical studies the majority of patients required 10 mg. Hypercholesterolimia: The dosage of atorvastatin in patients with homozygous FH is 10 to 80 mg daily. Atorvastatin should be used as an adjunct to other lipid-lowering treatments (e.g., LDL apheresis) in these patients or if such treatments are unavailable. Note: a 2.5/80 mg Caduet tablet is not available. In patients taking cyclosporine, therapy should be limited to atorvastatin 10 mg once daily. In patients taking clarithromycin, itraconazole or in patients with HIV taking a combination of ritonavir plus saquinavir or lopinavir plus ritonavir, for doses of atorvastatin exceeding 20 mg, appropriate clinical assessment is recommended to ensure that the lowest dose necessary of atorvastatin is employed. Consult a doctor before taking Caduet if you are pregnant or breastfeeding, if you are allergic to any of its ingredients, or if you have or have a history of liver disease, heart disease, kidney disease, hypothyroidism, or alcohol abuse. Do not stop taking this medication without consulting a doctor, especially if you are due to have major surgery or if you are suffering from serious illness or trauma, including sepsis, severe hypotension, uncontrolled seizures, or severe metabolic, endocrine or electrolyte disorders. Seek immediate medical attention if you develop: muscle pain/tenderness/weakness (especially with fever or unusual tiredness). Tell your doctor immediately if any of these highly unlikely but very serious side effects occur: irregular heartbeat, swelling of the hands or feet, worsening chest pain (attacks are more frequent, more severe, or last longer), yellowing of the eyes or skin, dark urine, severe fatigue, severe stomach/abdominal pain, persistent nausea, change in the amount of urine. Overdose Symptoms of Caduet overdose are unknown, but are likely to include dizziness, fainting, fast heartbeat, and other signs of severe hypotension. If you suspect overdose, contact your local poison control center or emergency room immediately.
