Glyburide, glibenclamideChemical NameGlibenclamide Ingredients Active Ingredients: Glibenclamide Non-Active Ingredients: Colloidal silicon dioxide, dibasic calcium phosphate, magnesium stearate, microcrystalline cellulose, sodium alginate, talc. In addition, the 2.5 mg contains aluminum oxide and FD&C Red No. 40 and the 5 mg contains aluminum oxide and FD&C Blue No. 1. Indications Glyburide is used in conjunction with exercise and diet to improve gycemic control in type II diabetes. How It Works It works by stimulating the release of your body's natural insulin. Dosage Tablets (1.25mg, 2.5 mg, 5mg) The recommended initial dose of Glyburide is 2.5 to 5 mg daily, administered with breakfast or the first main meal. Those patients who may be more sensitive to hypoglycemic drugs should be started at 1.25 mg daily. Transfer of patients from other oral anti-diabetic regimens to Glyburide should be done conservatively and the initial daily dose should be 2.5 to 5 mg. Patients Receiving Insulin: If the insulin dose is less than 20 units daily, substitution of Glyburide tablets 2.5 to 5 mg as a single daily dose is recommended. If the insulin dose is between 20 and 40 units daily, the patient may be placed directly on Glyburide Tablets 5 mg daily as a single dose. Maintenace: The usual maintenance dose is in the range of 1.25 to 20 mg daily, which may be given as a single dose or in divided doses (See Dosage Interval section). Dosage increases should be made in increments of no more than 2.5 mg at weekly intervals based upon the patient's blood glucose response. A maintenance dose of 5 mg of Glyburide tablets provides approximately the same degree of blood glucose control as 250 to 375 mg Chlorpropamide, 250 to 375 mg Tolazamide, 500 to 750 mg Acetohexamide, or 1000 to 1500 mg Tolbutamide. When transferring patients receiving more than 40 units of insulin daily, they may be started on a daily dose of Glyburide tablets 5 mg concomitantly with a 50% reduction in insulin dose. Daily doses of more than 20 mg are not recommended. Before using this medication, tell your doctor or pharmacist your medical history, especially of: alcohol use, severe breathing problems (obstructive lung disease, severe asthma), blood problems (anemia, vitamin B12 deficiency), electrolyte imbalance (hyponatremia), fertility problems (ovulation problems), certain hormonal conditions (adrenal/pituitary insufficiency, thyroid disease, syndrome of inappropriate antidiuretic hormone-SIADH), certain nervous system problem (autonomic neuropathy). Careful when using Glyburide, which can cause low PH in body tissue and blood resulting in potentially fatal consequences. Immediately stop taking medication if you experience any of the symptoms of lactic acidosis: severe drowsiness, chills, muscle pain, fast or difficult breathing, unusual tiredness, blue/cold skin, unusually slow or irregular heartbeat. These conditions most likely happen in people with kidney or liver conditions that may cause a low oxygen blood level or poor circulation, or in people who have had a severe congestive heart failure, recent heart attack, recent stroke, heavy alcohol use, liver disease, a severe loss of body fluids. Talk to your doctor about the birth control you are taking and whether it will be effective on this medication. Glucovance can cause anemia due to abnormal break-down of red blood cells, thus caution should be used in patients that have Glucose-6-phosphate dehydrogenase deficiency and a non-sulfonylurea based alternative should be used in its place. Taking Glucovance may cause increased risk of cardiovascular mortality; precautions should be used in patients that have serious cardiovascular issues. Hypoglycemia or hypoglycemic symptoms have occurred in patients using Glucovance, especially when strenuous exercise is not compensated by caloric supplementation, special attention is warranted to this concern. Liver or kidney insufficiency may elevate the concentration of medication creating increase risk of hypoglycemic reactions. People with kidney dysfunction with high serum creatinine levels or people that have kidney failure should not use this medication. Glucovance should be suspended for surgery. Do not drink alcohol while on this medication. This medicine may cause harm to an unborn baby. Before taking Glyburide, tell your doctor if you are pregnant or plan to become pregnant during treatment. Glyburide may pass into breast milk and may causes harm to a nursing baby. Consult your doctor before using this medication if you are pregnant or breast-feeding a baby. You should consult a doctor before using this medication if you are allergic to Glibenclamide or similar medicines, or any of the other ingredients of Glyburide. Drug Interactions Consult a doctor before taking Glyburide, if you are taking: (Bosentan (Tracleer), luoroquinolone type antibiotic, Levofloxacin and Ciprofloxacin and antidiabetic agents such as Glyburide, hydrochlorothiazide (Hydrodiuril) increase blood glucose levels, Salicylates, Sulfonamides, Chloramphenicol, Probenecid, coumarins, Monoamine Oxidase Inhibitors, and Beta Adrenergic Blocking Agents. Hyperglycemia and may lead to loss of control. These drugs include the thiazides and other diuretics, corticosteroids, phenothiazines, thyroid products, estrogens, oral contraceptives, phenytoin, nicotinic acid, sympathomimetics, calcium channel blocking drugs, miconazole and isoniazid. Cationic drugs like Amiloride, Digoxin, morphine, procainamide, quinidine, quinine, ranitidine, triamterene, trimethoprim, or vancomycin, fluconazole, ketoconazole, colesevelam, drugs that may affect the kidney?s ability to remove metformin from the body, cimetidine, cephalexin, diuretics: Furosemide, Thiazide diuretics such as Hydrochlorothiazide. Beta blockers such as Propranolol, Nifedipine, Furosemide, Linezolid, Moclobemide, phenelzine procarbazine, selegiline, Glyburide, Ciprofloxacin, Miconazole, Nonsteroidal Anti-Inflammatory Agents (NSAIDs) and other drugs that are highly protein bound, Salicylates, Sulfonamides, Chloramphenicol, Probenecid, Coumarins, Monoamine Oxidase Inhibitors, isocarboxazid, tranylcypromine, Niacin, phenothiazines (promethazine), phenytoin, probenecid, quinolone antibiotics and Beta-Adrenergic Blocking agents, insulin, metformin, aspirin or aspirin-like drugs, salicylates, NSAIDs such as Ibuprofen, Phenylbutazone), azole antifungals , fluconazole, birth control pills, "blood thinners" (warfarin), calcium channel blockers (diltiazem), chloramphenicol, clofibrate, colesevelam, corticosteroids (prednisone), decongestants (pseudoephedrine), diazoxide, diet pills, epinephrine, estrogens, furazolidone, linezolid, moclobemide, phenelzine procarbazine, selegiline, rasagiline, (rifampin, sulfonamides, sulfa antibiotics, thyroid drugs, "water pills" (diuretics such as hydrochlorothiazide, furosemide), fenugreek or ginsing. Also speak with your doctor about any drugs that can affect your blood sugar levels, making it more difficult to control your blood sugar levels Before using this Glucovance, tell your doctor or pharmacist about all prescription and nonprescription medications, including herbal products, minerals and vitamins you may be taking. Other medications may affect how Glucovance works or may cause other unforeseen complications. Adverse/Side Effects It is common that Glyburide causes the following minor side effects: nausea, heartburn, stomach fullness, and weight gain. If these side effects persist or become overly bothersome contact your doctor or health-care professional. Infrequent side effects: sore throat or fever, easy bleeding or bruising, stomach pain, yellowing eyes or skin, dark urine, unusual tiredness or weakness, unusual or sudden weight gain, mental/mood changes, swelling of the hands or feet, seizures, hypoglycemia: This effect may occur if you do not consume enough calories (from food, juices, fruit, etc).
