How It Works Voltage-gated sodium channels are the molecular pores that allow brain cells (neurons) to generate action potentials, the electrical events that allow neurons to communicate over long distances. After the sodium channels open to start the action potential, they inactivate, essentially closing the channel. Carbamazepine stabilizes the inactivated state of sodium channels, meaning that fewer of these channels are available to subsequently open, making brain cells less excitable (less likely to fire). Since seizures are caused by uncontrolled firing of neurons, carbamazepine works to prevent them from happening. Dosage Available tablet sizes: 100 mg, 200 mg, 400 mg Available extended-release (XR) tablet sizes: 200 mg, 400 mg Adults and children over age 12: The standard initial dose is 200 mg of either regular or extended-release (XR) tablets, two times a day. Dosage should then be increased by 200 mg/day at weekly intervals, up to the lowest effective maintenance dosage (usually between 800 and 1200 mg daily). Regular tablets should be taken three or four times daily, while XR tablets should only be taken twice daily. Children aged 12-15 should not take more than 1000 mg daily, while patients older than 15 should not take more than 1200 mg daily, although daily doses of up to 1600 mg have been used in rare cases. Children ages 6-12: The usual starting dosage for children is 100 mg twice daily or either regular or XR tablets. Dosage should then be increased by 100 mg/day at weekly intervals. XR tablets should be taken twice a day, while regular tablets should be taken three or four times daily. The usual maintenance dosage is from 400 to 800 mg daily, and not exceeding 1000 mg daily. Children under age 6: Initial dosage is from 10-20 mg/kg/ two or three times per day of regular tablets. Dosage should be increased gradually, on a weekly basis. Maintenance dosages are usually below 35 mg/kg/day, divided into three or four doses. Neuralgia: The initial dosage for treating neuralgia is 100 mg taken twice daily, either regular tablets or XR. This dosage may be increased at a rate of up to 200 mg/day, using increments of 100 mg every 12 hours, until freedom from neuralgia pain is achieved. The usual maintenance dosage is from 400 to 800 mg daily, though 200 mg daily has been known to be effective. Dosages should not exceed 1200 mg daily. At least once every 3 months throughout the treatment period, attempts should be made to reduce the dose to the minimum effective level or even to discontinue the drug. Notes on Tegretol-XR: Tegretol-XR is an extended-release formulation for twice-a-day administration. When converting patients from Tegretol conventional tablets to Tegretol-XR, the same total daily mg dose of Tegretol-XR should be administered. Tegretol-XR tablets must be swallowed whole and never crushed or chewed. Tegretol-XR tablets should be inspected for chips or cracks. Damaged tablets, or tablets without a release portal, should not be consumed.

This drug may make you sensitive to sunlight – avoid prolonged sun exposure, tanning booths or sunlamps while taking it. Adverse/Side Effects Nausea, vomiting, dizziness, drowsiness, or unsteadiness may occur as your body adjusts to this medication. If any of these common side effects persist or worsen, notify your doctor or pharmacist promptly. Tell your doctor immediately if any of these unlikely but serious side effects occur: chest pain, swelling of the ankles/feet, trouble breathing, fatigue, fast/slow/irregular heartbeat, persistent or severe headache, fainting, trouble urinating, change in the amount of urine, decreased sexual ability, unusual eye movements (nystagmus), vision changes, hearing problems, mental/mood changes, pain/redness/swelling of the arms or legs, numbness/tingling of the hands/feet, sun sensitivity, joint pain, hair loss. Tell your doctor immediately if any of these rare but very serious side effects occur: persistent nausea or vomiting, severe stomach/abdominal pain, yellowing eyes and skin, dark urine, swollen glands, persistent mouth sores. A very serious allergic reaction to this drug is unlikely, but seek immediate medical attention if it occurs. Symptoms of a serious allergic reaction may include: rash, itching, swelling, severe dizziness, trouble breathing. Overdose Symptoms of overdose include slowed breathing, loss of consciousness, muscle twitching, uncontrolled movements, and very fast heartbeat. If you suspect overdose, contact your local poison control center or emergency room immediately.
