Zyloprim, allopurinol

Chemical Names
Allopurinol

Ingredients
Active Ingredients: allopurinol
Non-Active Ingredients: lactose, magnesium stearate, potato starch, povidone, corn starch, FD&C Yellow No. 6 Lake.

Indications
Allopurinol is used to treat gout caused by excessive levels of uric acid in the blood (hyperuricemia). Similarly, it is used to treat acute attacks of gout, erosive destructive gout joint disease, uric acid deposits in tissues, gout kidney disease, and uric acid stones. Allopurinol also is used to prevent elevation of blood uric acid in patients undergoing chemotherapy for the treatment of certain cancers and in patients with recurrent calcium kidney stones and elevated uric acid levels.

How It Works
Allopurinol prevents the production of uric acid by blocking the activity of the enzyme that converts purines to uric acid. Uric acid levels usually begin to fall within 2-3 days of starting treatment and return to their original levels within 7-10 days after allopurinol is stopped.

Dosage
Available tablet sizes: 100 mg, 300 mg

For gout: allopurinol should be initiated at a dose of 100 to 200 mg daily and increased by 100 mg daily at weekly intervals, until a serum uric acid concentration of about 360 µmol/L (6 mg/dL) or less is attained, or until the maximum recommended dosage of 800 mg/day is reached. After serum urate concentrations are controlled, it may be possible to reduce dosage; the average maintenance dosage is 200 to 300 mg/day for patients with mild gout.

Renal impairment: with a creatinine clearance of 0.33 to 0.17 mL/s, a daily dosage of 200 mg of allopurinol is suitable. When the creatinine clearance is less than 0.17 mL/s, the daily dosage should not exceed 100 mg.

Prevention of uric acid nephropathy during agressive therapy of neoplastic disease: treatment with 600 to 800 mg daily for 2 or 3 days prior to chemotherapy or irradiation is advisable. When allopurinol is used with mercaptopurine or azathioprine, dosage of the latter drugs must be reduced.

Prophylaxis of renal calcium lithiasis: 200 to 300 mg daily. In some patients, the maintenance dose is 100 to 200 mg daily.
