Ingredients
Active Ingredients: Digoxin
Non-Active Ingredients: Corn starch, croscarmellose sodium, microcrystalline cellulose, pregela-tinized starch, lactose monohydrate and anhydrous lactose, silicon dioxide and stearic acid

Indications
Lanoxin is indicated in the treatment of mild to moderate heart failure, to control ventricular response rate in patients with chronic atrial fibrillation (atrial fibrillation, atrial flutter, and atrial tachycardia) and to increase the strength and vigor of heart contractions.

How It Works
Digoxin increases the force of contraction of the muscle of the heart by inhibiting the activity of an enzyme that controls movement of calcium, sodium and potassium into the heart muscle. Calcium influences the force of contraction. Digoxin increases calcium in heart muscle and therefore increases the force of heart contractions. Digoxin also slows electrical conduction between the atria and the ventricles of the heart and is useful in treating abnormally rapid atrial rhythms such as atrial fibrillation, atrial flutter, and atrial tachycardia. Treating an irregular heartbeat is important because it will decrease the risk of blood clots, and may reduce your risk for a heart attack or stroke.

Dosage
Tablets (0.25 mg)

Take this medication orally once daily or as directed by your doctor. The starting dose of Digoxin is 0.25 mg per day for an adult under 70 with proper kidney function. If the patient is over 70 or has decreased renal function then the starting dosage is halved. The dose may be increased every two weeks to achieve the desired response. Do not increase your dose more than once every 14 days.
 
Try to take it at the same time each day. You should take this medication at least 2 hours before or after eating. It is important to continue taking this medication even if you feel well. Do not stop taking this medication without consulting your doctor. Some conditions may become worse when the drug is abruptly stopped. Your dose may need to be gradually decreased.

Precautions
You should not use this medication if you are allergic to Digoxin or any of the other ingredients in it. Tell your doctor your whole medical history before taking this drug, especially: Kidney impairment, or thyroid problems (underactive or overactive). If you’re having surgery during your treatment with Digoxin, tell the surgeon ahead of time. Do not stop using the medication before surgery unless your surgeon tells you to. Digoxin is primarily eliminated from the body by the kidneys; and therefore, patients with impaired kidney function require smaller than usual doses of Digoxin. Digoxin stays in the body for a long time and if the patient has impaired kidney function, Digoxin can build up in toxic quantities.
 
The balance of the natural minerals in your blood (calcium, magnesium, potassium) will affect how this drug works. Certain drugs such as "water pills" (diuretics) may affect balance of minerals in your body. Tell your doctor if you are taking diuretics or if you have a history of mineral imbalance. You may want to take a nutritional supplement or follow a special diet while on Lanoxin. It may be desirable to reduce the dose of Digoxin a few days prior to having an electrical cardioversion operation. Physicians must consider the consequences of increasing the ventricular response if Digoxin is withdrawn.
 
It is unknown whether this medication can harm an unborn baby. Before taking Digoxin, tell your doctor if you are pregnant or plan to become pregnant during treatment. Digoxin does pass into breast milk and may cause harm to a nursing baby. Consult your doctor before using this medication if you are breast-feeding a baby.
 
This drug may make you dizzy or drowsy. You should not drive, use machinery, or do any activity that requires alertness until you are sure you can perform such activities safely.


Drug Interactions
Do not start, stop or change the dosage of any medicine before checking with your doctor first. Before using this medication, tell your doctor or pharmacist about all prescription and nonprescription medications, including herbal products and vitamins you may use. Erythromycin and clarithromycin (and possibly other macrolide antibiotics) and tetracycline may increase Digoxin absorption in patients, so that digitalis intoxication may result. Antacids, kaolin-pectin, sulfasalazine, neomycin, cholestyramine, certain anticancer drugs, and metoclopramide may interfere with intestinal digoxin absorption, resulting in unexpectedly low serum concentrations.

Drugs such as Calan, Verelan, Isoptin, Covera-HS (verapami), Quinaglute, Quinide (quinidine), Cordarone (amiodarone), Indocin (indomethacin), Xanax, Niravam (alprazolam) , Aldactone (spironolactone), and Sporanox (itaconazolre)  can increase Digoxin levels and the risk of toxicity. The co-administration of Digoxin and beta-blockers like propranolol and others or calcium channel blockers (verapamil), which also reduces heart rate, can cause serious slowing of the heart rate. Saquinavir (invirase) and Norvir (ritonavir) increase the amount of Digoxin in the body and may cause Digoxin toxicity.

Other medications can affect the removal of Lanoxin from your body, which may affect how Lanoxin works. Examples include Amiodarone, Azole antifungals (such as Itraconazole, Ketoconazole), cyclosporine, HIV medications (such as Ritonavir, Saquinavir), macrolide antibiotics (such as Clarithromycin, Erythromycin), Propafenone, Ranolazine, Rifampin, and St. John's wort, among others. Ask your pharmacist about using this product safely. Also if you are taking Cholestyramine, Colestipol, or Psyllium, wait 2 hours after taking your Digoxin dose before taking any of these drugs. If you are taking antacids, kaolin-pectin, milk of magnesia, Metoclopramide, Sulfasalazine, or Aminosalicylic acid, take them as far apart from Digoxin as possible.

Adverse/Side Effects
Common side effects include: nausea, vomiting, headache, loss of appetite, and diarrhea. If any of these effects persist or worsen, tell your doctor or pharmacist promptly. Many of these side effects are dose dependent and happen when blood levels are over the therapeutic range.
Remember that your doctor has prescribed this medication because he or she has judged that the benefit of treatment outweight the risks or side effects. Many people using this medication do not have serious side effects. Tell your doctor immediately if any of these unlikely but serious side effects occur: heart block, heart rate increase or decrease, weakness, mental/mood changes, vision changes (such as blurred or yellow/green vision), enlarged/tender breasts in men. Though this medication is used to treat a certain type of irregular heartbeat, it may infrequently cause other types of irregular heartbeats. Tell your doctor immediately if you notice any unusually fast/slow/irregular heartbeat. A very serious allergic reaction to this drug is rare. Get emergency medical help if you have any of these signs of an allergic reaction: rash, itching/swelling (especially of the face/tongue/throat), severe dizziness, trouble breathing. This is not a complete list of possible side effects. If you notice other effects not listed above, contact your doctor or pharmacist.

Overdose
Symptoms of overdose include severe nausea/vomiting/diarrhea, dizziness, fainting, very fast/slow/irregular heartbeat. If you suspect overdose, contact your local poison control center or emergency room immediately.
