Ingredients
Active Ingredients: Efavirenz

Non-Active Ingredients: Croscarmellose sodium, hydroxypropyl cellulose, lactose monohydrate, magnesium stearate, microcrystalline cellulose, and sodium lauryl sulfate. The film coating contains Opadry Yellow and Opadry Clear. The tablets are polished with carnauba wax and printed with purple ink, Opacode WB.

Indications
Sustiva is used in combination with other anti-retroviral drugs in the treatment of patients with Human Immunodeficiency Virus type 1, (HIV-1).

How It Works
Efavirenz is an antiviral medication that prevents human immunodeficiency virus (HIV) cells from multiplying in your body. It is usually given in the first wave of treatment against HIV.

Dosage
Tablets (200 mg, 600 mg)
The recommended dosage of Sustiva is 600 mg orally, once per day, normally taken at bedtime. Sustiva is usually taken with a protease inhibitor and/or nucleoside analogue reverse transcriptase inhibitors (NRTIs). Sustiva should be taken on an empty stomach, combining the drug with food may lead to an increased frequency of side effects.

Precautions
Sustiva is contraindicated in people who hyper sensitive to efavirenz, people who are allergic to it, similar medicines, or any of the other ingredients in Sustiva.

Speak to a doctor if you have a history of the following conditions:  mental/mood conditions (such as depression, thoughts of suicide, psychosis), liver problems or liver disease, hepatitis B or C, high cholesterol or triglycerides, or if you have ever taken delavirdine or nevirapine and they were not effective in treating your condition.

This medicine will cause harm to an unborn baby especially if taken in the first trimester. Before taking Sustiva, tell your doctor if you are pregnant or plan to become pregnant during treatment. Sustiva passes into breast milk and may harm a nursing baby, you should not breastfeed while on it. You should not breast feed if you have HIV or AIDS at all, as the virus can be transmitted through breast milk. Consult your doctor before using this medication if you are pregnant. HIV can be passed to the baby if the mother is not properly treated during pregnancy. Take all of your HIV medications as directed to control your infection while you are pregnant.

A rash or skin irritation may be more common in children taking Efavirenz. Report any rash immediately as it may evolve into a serious complication.

Sustiva may cloud your thought process or reaction speed. Be careful if you drive or do anything that requires alertness. Drinking alcohol can increase the side effects of Sustiva.

This medication will not prevent HIV from passing to other people during intercourse or through blood. Avoid unprotected sex or sharing razors or toothbrushes etc.

Drug Interactions
Consult a doctor before taking Sustiva, if you are taking: cisapride, pimozide, midazolam, triazolam, or ergot medicines such as dihydroergotamine, ergonovine, ergotamine, or methylergonovine. These drugs can cause serious medical problems and potentially even death, they should in no circumstances be used with Sustiva.

Before taking Efavirenz, tell your doctor if you are using any of the following drugs: Itraconazole, St. John's wort, Voriconazole, a blood thinner such as warfarin, a cholesterol medication such as Lipitor or Zocor, any antibiotics such as Clarithromycin, Rifabutin, or Rifampin, heart or blood pressure medications such as Amlodipine, Diltiazem, Felodipine, Nicardipine, Nifedipine, or Verapamil, other HIV medicines such as Amprenavir, Indinavir, Lopinavir/Ritonavir, saquinavir, or seizure medications such as phenytoin or carbamazepine.

Sustiva is almost always prescribed with other HIV or AIDS medications. Sustiva should not be administered alone as it may cause the virus to quickly develop a resistance. The choice of antiretroviral agents to be used in combination with Efavirenz should take into consideration the potential for viral cross-resistance.

Coadministration of Sustiva with Atripla (Efavirenz 600 mg/Emtricitabine 200 mg/Tenofovir Disoproxil Fumarate 300 mg) is not recommended, since Efavirenz is already one of its active ingredients.

If Sustiva is co-administered with Voriconazole, the voriconazole maintenance dose should be increased to 400 mg every 12 hours and the Sustiva dose should be decreased to 300 mg per day.

Before using this Sustiva, tell your doctor or pharmacist about all prescription and nonprescription medications, including herbal products and vitamins you may be taking. Other medications may affect how Sustiva works or might cause other unforeseen complications.

Adverse/Side Effects
It is common that Sustiva causes the following minor side effects: mild nausea, vomiting, or stomach pain, diarrhea or constipation cough, blurred vision, headache, dizziness, tired feeling, trouble concentrating, problems with balance or coordination, muscle or joint pain, sleep problems (insomnia), unusual dreams,or changes in the shape or location of body fat (especially in your arms, legs, face, neck, breasts, and waist). If these side effects persist or become overly bothersome contact your doctor or health-care professional.

If you experience any of the following serious side effects you should contact your doctor immediately: fever, sore throat, and headache with a severe blistering, peeling, and red skin rash, nausea, stomach pain, loss of appetite, dark urine, clay-colored stools, jaundice (yellowing of the skin or eyes), fever, chills, body aches, flu symptoms, or any other signs of new infection.

Efavirenz may also cause serious psychiatric symptoms including: confusion, suicidal thoughts, severe depression, extreme fear, hallucinations, aggression, or other unusual behavior. Contact your doctor at once if you're experiencing any of these side effects, especially if you have a history of psychiatric disorders.

A very serious allergic reaction to this drug is unlikely. Get emergency medical help if you have any of these signs of an allergic reaction: rash, itching/swelling (especially of the face/tongue/throat), severe dizziness, or trouble breathing. This may not be a complete list of side effects and may be subject to change, consult with your doctor or pharmacist if you are experiencing any worrisome side effects.

Overdose
If you suspect overdose, contact your local poison control center or emergency room immediately. Symptoms of overdose include confusion, lack of balance or coordination, severe mood or behavior changes, or thoughts of suicide.
