Ziagen, abacavirChemical Namesabacavir Ingredients Active ingredients: abacavir Non-active ingredients: colloidal silicon dioxide, magnesium stearate, microcrystalline cellulose, and sodium starch glycolate. Indications Ziagen tablets and oral solution, in combination with other antiretroviral agents, are indicated for the treatment of human immunodeficiency virus (HIV-1) infection. How It Works Ziagen prevents HIV from altering the genetic material of healthy T-cells. This prevents the cells from producing new virus and decreases the amount of virus in the body. Dosage Oral Tablets (300 mg) Adult Patients The recommended oral dose of Ziagen for adults is 600 mg daily, administered as either 300 mg twice daily or 600 mg once daily, in combination with other antiretroviral agents. Hepatic Impairment: The recommended dose of ZIAGEN in patients with mild hepatic impairment (Child-Pugh score 5 to 6) is 200 mg twice daily. Pediatric Patients The recommended oral dose of ZIAGEN Oral Solution in HIV-1-infected pediatric patients aged 3 months and older is 8 mg/kg twice daily (up to a maximum of 300 mg twice daily) in combination with other antiretroviral agents. Weighing greater than or equal to 14 kg for whom a solid dosage form is appropriate. Before prescribing ZIAGEN Tablets, children should be assessed for the ability to swallow tablets. If a child is unable to reliably swallow ZIAGEN Tablets, the oral solution formulation should be prescribed. Lactic acidosis and severe hepatomegaly with steatosis, including fatal cases, have been reported with the use of nucleoside analogues alone or in combination, including abacavir and other antiretrovirals. A majority of these cases have been in women. Obesity and prolonged nucleoside exposure may be risk factors. Redistribution/accumulation of body fat including central obesity, dorsocervical fat enlargement (buffalo hump), peripheral wasting, facial wasting, breast enlargement, and “Cushingoid appearance” have been observed in patients receiving antiretroviral therapy. Careful caution should be exercised when administering Ziagen to any patient with known risk factors for liver disease. Immune reconstitution syndrome has been reported in patients treated with combination antiretroviral therapy, including Ziagen. Studies have shown an increased risk of myocardial infarction. Abacavir induced chromosomal aberrations both in the presence and absence of metabolic activation in an in vitro cytogenetic study in human lymphocytes. There is a chance of an increase in the incidence of malignant and non-malignant tumors. There are no adequate and well-controlled studies in pregnant women. Ziagen should be used during pregnancy only if the potential benefits outweigh the risk. The Centers for Disease Control and Prevention recommend that HIV-1-infected mothers not breastfeed their infants to avoid risking postnatal transmission of HIV-1 infection. Although it is not known if abacavir is excreted in human milk and should be used with extreme caution. Ziagen can be an extremely dangerous medication and should only be used as prescribed by your doctor. Drug Interactions Tell your doctor of all nonprescription and prescription medication you may use, especially of: other antiretrovirals, drugs that cause liver problems (e.g., isoniazid), lamivudine and tenofovir. Do not start or stop any medicine without doctor or pharmacist approval. Adverse/Side Effects Headache and trouble sleeping may occur. If these effects persist or worsen, notify your doctor. Very unlikely but report promptly: dark urine, yellowing of the eyes or skin, rapid breathing, change in amount of urine. Changes in body fat may occur while you are taking this medication (e.g., increased fat in the upper back and stomach areas, decreased fat in the arms and legs). The cause and long-term effects of these changes are unknown. Discuss the risks and benefits of therapy with your doctor, as well as the possible role of exercise to reduce this side effect. Infrequently, this drug can cause severe (sometimes fatal) allergic reactions, liver problems, and blood acid-base balance (lactic acidosis) problems. If you develop symptoms such as rash, fever, fatigue, difficult or rapid breathing, cough, throat inflammation, dark urine, yellowing of the skin or eyes, drowsiness, muscle aches, stomach/abdominal pain, nausea, vomiting, or diarrhea, immediately stop using this medication and notify your doctor. In the unlikely event you have an allergic reaction to this drug, seek immediate medical attention. Do not restart this medication if an allergic reaction to this drug has occurred because more severe, life-threatening problems could occur (including low blood pressure and death). Also, if an allergic reaction is only suspected, this medication should never be restarted. (See precautions regarding stopping and starting this medication.) It is not known how long this drug will benefit those with HIV infection. Overdose If overdose is suspected, contact your local poison control center or emergency room immediately. Storage Instructions Store capsules at room temperature between 68 and 77 degrees F (20-25 degrees C) away from light and moisture.
