Indications Sinemet is indicated in the treatment of the symptoms of idiopathic Parkinson's disease (paralysis agitans), post-encephalitic parkinsonism, and symptomatic parkinsonism which may follow injury to the nervous system by carbon monoxide intoxication and/or manganese intoxication. How It Works Evidence suggests that symptoms of Parkinson's disease are related to depletion of dopamine in the corpus striatum. Administration of dopamine is ineffective in the treatment of Parkinson's disease apparently because it does not cross the blood-brain barrier. However, levodopa, the metabolic precursor of dopamine, does cross the blood-brain barrier, and presumably is converted to dopamine in the brain. This is thought to be the mechanism whereby levodopa relieves symptoms of Parkinson's disease. Dosage Available tablet sizes (carbidopa/levodopa): 10mg/100mg, 25mg/100mg, 25mg/250mg, 50mg/200mg The optimum daily dosage of Sinemet must be determined by careful titration, and adjusted according to the desired therapeutic response. Dosage is best initiated with one 25/100mg tablet of Sinemet three times a day. This dosage schedule provides 75 mg of carbidopa per day. Dosage may be increased by one tablet every day or every other day, as necessary, until a dosage of eight tablets of Sinemet 25-100 a day is reached. Levodopa must be discontinued at least twelve hours before starting Sinemet (carbidopa/levodopa). A daily dosage of Sinemet should be chosen that will provide approximately 25% of the previous levodopa dosage. Take Sinemet by mouth on an empty stomach at least 1 hour before or 2 hours after eating. If you are also taking iron salts (e.g., ferrous sulfate), do not take them within 2 hours before or 2 hours after taking Sinemet. Do not suddenly stop taking Sinemet. Some conditions may become worse when Sinemet is suddenly stopped. Your dose may need to be slowly lowered by your doctor to avoid side effects. Neuroleptic malignant syndrome (NMS) is a possibly fatal syndrome that can be caused by Sinemet. Symptoms may include fever; stiff muscles; confusion; abnormal thinking; fast or irregular heartbeat; and sweating. Contact your doctor at once if you have any of these symptoms. Overdose Symptoms of overdose include severe dizziness, irregular or fast heartbeat, increased blinking/eyelid spasms (blepharospasms).
