Stalevo, carbidopa/levodopa/entacaponeChemical Namescarbidopa/levodopa/entacapone Ingredients Active ingredients: carbidopa, levodopa and entacapone Non active: corn starch, croscarmellose sodium, glycerol 85%, hypromellose, magnesium stearate, mannitol, polysorbate 80, povidone, sucrose, red iron oxide, and titanium dioxide. Stalevo 50, Stalevo 100, and Stalevo 150 also contain yellow iron oxide. Indications Stalevo is indicated in the treatment of symptoms of Parkinson’s disease. How It Works Parkison’s disease is belived to be due to a lack of dopamine in the brain. Levodopa is converted into dopamine in the brain, leading to the relief of Parkison’s disease symptoms. Both carbidopa and entacapone are given with levodopa to increase the amout of levodopa that is converted into dopamine. Dosage Available tablet sizes (carbidopa/levodopa/entacapone): 12.5/50/200mg, 25/100/200mg, 37.5/150/200mg The optimum daily dosage of Stalevo must be determined by careful titration in each patient. The maximum recommended daily dose of Stalevo 50, Stalevo 75, Stalevo 100, Stalevo 125 and Stalevo 150, defined by the maximum daily dose of entacapone, is eight tablets per day. Because there is limited experience with total daily doses of carbidopa greater than 300mg, the maximum recommended daily dose of Stalevo 200 is six tablets per day. Precautions Consult a doctor before taking Stalevo if you are pregnant or breastfeeding, if you are allergic to any of its ingredients, or if you have or have a history of mental or mood disorders, cardiovascular disease, asthma, kidney disease, liver disease, endocrine disease, heart attacks, arrhythmia, glaucoma, or biliary obstruction. Stalevo may cause a false-positive reaction for urinary ketone bodies when a test tape is used for determination of ketonuria. Drug Interactions Caution should be exercised when the following drugs are administered: anti-hypertensive agents, MAO inhibitors, tricyclic antidepressants, dopamine D2 receptor antagonists (e.g., phenothiazines, butyrophenones, risperidone), isoniazid, phenytoin, papaverine, iron salts, metoclopramide, drugs known to interfere with biliary excretion, glucuronidation, and intestinal beta-glucuronidase (probenecid, cholestyramine, erythromycin, rifampicin, ampicillin and chloramphenicol), pyridoxine, isoproterenol, epinephrine, norepinephrine, dopamine, dobutamine, alpha-methyldopa, apomorphine, isoetherine, and bitolterol. Adverse/Side Effects Diarrhea, nausea, vomiting, headache, dizziness, drowsiness, stomach upset, loss of appetite, dry mouth, or difficulty sleeping may occur. If any of these effects persist or worsen, notify your doctor or pharmacist promptly. Tell your doctor immediately if any of these serious side effects occur: involuntary movements (especially twitching of the eyelid), fainting or falling, confusion, difficulty swallowing, stomach/abdominal pain, vision changes (including blurred vision), hallucinations, chest pain, black/tarry stools, muscle pain, change in the amount of urine, mental/mood changes or depression (sometimes severe, including rare thoughts of suicide), persistent sore throat, high fever, severe muscle stiffness, fast heartbeat, fast breathing, increased sweating. This medication may cause the saliva, urine, or sweat to turn a dark color. This is harmless; however, it may discolor clothing. A serious allergic reaction to this drug is unlikely, but seek immediate medical attention if it occurs. Symptoms of a serious allergic reaction include: rash, itching, swelling, severe dizziness, trouble breathing. If you notice other effects not listed above, contact your doctor or pharmacist. The addition of carbidopa to levodopa reduces the peripheral effects (nausea, vomiting) due to decarboxylation of levodopa; however, carbidopa does not decrease the adverse reactions due to the central effects of levodopa. Because carbidopa as well as entacapone permits more levodopa to reach the brain and more dopamine to be formed, certain adverse CNS effects, e.g., dyskinesia (involuntary movements) may occur at lower dosages and sooner with levodopa preparations containing carbidopa and entacapone than with levodopa alone. Cases of a symptom complex resembling the neuroleptic malignant syndrome characterized by elevated temperature, muscular rigidity, altered consciousness, and elevated CPK have been reported in association with the rapid dose reduction or withdrawal of other dopaminergic drugs. Cases of retroperitoneal fibrosis, pulmonary infiltrates, pleural effusion, and pleural thickening have been reported in some patients treated with ergot derived dopaminergic agents. Overdose Symptoms of overdose include severe dizziness, irregular or fast heartbeat, increased blinking/eyelid spasms (blepharospasms). If you suspect overdose, contact your local poison control center or emergency room immediately. Storage Instructions No special storage requirements are necessary. This medication must not be used after the expiry date indicated on the carton. |
