Gel: Purified water, alcohol, triethanolamine and carbomer 934P

Cream: purified water, propylene glycol, stearyl alcohol, white ceresin wax, mono- and di-glycerides, hypromellose 2208 (4000 cps), sodium lauryl sulfate, methylparaben, edetate di-sodium and tertiary-butylhydroquinone

Patch: Alcohol USP gelled with hydroxypropyl cellulose, an ethylene-vinyl acetate copolymer membrane, and an adhesive formulation of light mineral oil, and polyisobutylene. A protective liner of siliconized polyester film is attached to the adhesive surface.

Indications
Estrace is used to treat postmenopausal woman who have thinning in the lining of the vagina due to the decrease in estrogen production. This causes Vaginitis, meaning inflammation of the vagina, aside from atrophic vaginitis. This leads to various other conditions, genital dryness, itching, burning, painful sex, itchy vulva, feeling of pressure, yellow malodorous urinary discharge, painful urination, frequent urination, bladder emptying while sleeping, cystitis, urinary tract infection, urinary stress incontinence. In addition estradiol is used for hot flashes.

Additional gel indications: Muscle and nerve function affected by menopause.

Additional patch indications: Hypo-estrogenism due to hypogonadism, castration, or primary ovarian failure. Prevention of postmenopausal osteoporosis.

How It Works
Estrace, provides systemic estrogen replacement therapy by releasing estradiol, the major estrogenic hormone secreted by human ovaries. Endogenous estrogens are largely responsible for the development and maintenance of the female reproductive system and secondary sexual characteristics.

Tablets:
Estrace is gently inserted into the vagina as far as it can comfortably go without force, using the supplied applicator.

Thinning of the lining of the vagina and vaginitis initial dose: Estrace tablet, inserted vaginally, once daily for two weeks. It is advisable to have the patient administer treatment at the same time each day.

The Maintenance dose: One Estrace tablet, inserted vaginally, twice weekly.

Gel:
EstroGel 1.25 g is the single approved dose for the treatment of moderate to severe vasomotor symptoms and for the treatment of moderate to severe symptoms of vulvar and vaginal atrophy due to menopause. The lowest effective dose of EstroGel for these indications will be determined by doctor. This medication is usually applied in a thin layer once daily according to product instructions or as directed by your doctor. If you shower or bathe before applying this medication, allow the skin to dry completely before use. Apply this drug to clean, unbroken skin. Do not apply to irritated skin. Do not apply this medication to the face or breasts or in the vagina. It is not necessary to rub this medicine into the skin. Allow this medication to dry on your skin before dressing. Wash your hands with soap and water after each application.

Cream:
Use of Estrace (estradiol vaginal cream, USP, 0.01%), alone or in combination with a progestin, should be limited to the shortest duration consistent with treatment goals and risks for the individual woman. Patients should be re-evaluated periodically as clinically appropriate (3-month to 6-month intervals) to determine if treatment is still necessary. For treatment of vulvar and vaginal atrophy associated with the menopause, the lowest dose and regimen that will control symptoms should be chosen and medication should be discontinued as promptly as possible. The usual dosage range is 2 to 4 g (marked on the applicator) daily for one or two weeks, then gradually reduced to one half initial dosage for a similar period. A maintenance dosage of 1 g, one to three times a week, may be used after restoration of the vaginal mucosa has been achieved.

Patch: The adhesive side of the Estraderm system should be placed on a clean, dry area of the skin on the trunk of the body (including the buttocks and abdomen). The site selected should be one that is not exposed to sunlight. Estraderm should not be applied to the breasts. The Estraderm system should be replaced twice weekly. The sites of application must be rotated, with an interval of at least 1 week allowed between applications to a particular site. The area selected should not be oily, damaged, or irritated. The waistline should be avoided, since tight clothing may rub the system off. The system should be applied immediately after opening the pouch and removing the protective liner. The system should be pressed firmly in place with the palm of the hand for about 10 seconds, making sure there is good contact, especially around the edges. In the unlikely event that a system should fall off, the same system may be reapplied. If necessary, a new system may be applied. In either case, the original treatment schedule should be continued.

Estraderm is currently available in two dosage forms - 0.05 mg and 0.1 mg. Patients should be started at the lowest dose. The lowest effective dose of Estraderm has not been determined.
 

Precautions
Estrace is contraindicated in people who have experienced a hyper-senativity to its ingredients, and patients with breast tumors, uncontrolled cell growth, abnormal genital bleeding conditions, heme-production vein inflammation from blood clotting, and formation of blood clots in blood vessels.

Speak to a doctor about using estradiol if you have the following conditions: asthma, yellowing skin, epilepsy, migraine, cardiac problems or renal dysfunction.

Long-term use may increase chance of developing tumors in cervix, vagina, liver and uterine lining. It has been proven that hormones and birth control increase the chance of vascular disease like inflammation due to blood clotting, pulmonary embolism, stroke, and heart attacks. There is an increased risk of gall bladder disease in woman taking estrogen.

Woman taking estrogen will get increased blood pressure, which will return to normal once they discontinue product. When taking estrogens there is a worsening of glucose tolerance, for this reason diabetic patients should take special precautions.

Estrogens can lead to severe hyper-calcemia in some patients. There is a risk of trauma induced by Vagifem applicator; this can result in atrophic vaginal mucosa, a condition occurring in some women, typically postmenopausal women, in which there is significant thinning of the mucosa of the vagina. There is a significant possibility of uterine bleeding on Estrace. There is also a possibility of overgrowth in the lining of the uterus. Vaginal infections are more common in postmenopausal woman.

Young woman or men should not take Estrace, unless directed so by a doctor. Additional estrogel precaution: if someone comes into contact with gel or cream accidentally they should immediately wash it off, especially important for men and children!

This medicine will cause harm to an unborn baby. Before taking Estrace, tell your doctor if you are pregnant or plan to become pregnant during treatment. Estrace passes into breast milk and may causes harm to a nursing baby. Consult your doctor before using this medication if you are pregnant or breast-feeding a baby.

You should consult a doctor before using this medication if you are allergic to Estradiol or similar medicines, or any of the other ingredients of Estrace.

Drug Interactions
Consult a doctor before taking Estrace, if you are taking: warfarin, corticosteroids (prednisone), raloxifene, tamoxifen, drugs affecting liver enzymes that remove estrogens from your body (such as St. John's wort), phenobarbital, phenytoin, carbamazepine, rifamycins including rifabutin, azole antifungals including itraconazole, and macrolide antibiotics including erythromycin.

All endocrine and liver function tests will be affected by estrogens.

Before using Estrace, tell your doctor or pharmacist about all prescription and nonprescription medications, including herbal products and vitamins you may be taking. Other medications may affect how Estrace works or might cause other unforeseen complications.


Adverse/Side Effects

The common side effects with Estrace use: vaginal spotting, vaginal discharge, allergic reaction and skin rash.

Infrequent side effects: headache, abdominal pain, upper respiratory tract infection, inflammation caused by yeast infection, back pain, upper respiratory tract infection, itchy inflamed genitals, allergy, bronchitis, dyspepsia, blood in the urine, hot flashes, insomnia, pain, sinusitis, vaginal discomfort, vaginitis, nipple discharge, skin inflammation, lumps or nodules caused by skin reaction.

Rare side effects: breast cancer, cancer of the uterus, stroke, heart attack, blood clots, dementia, ovarian cancer, high blood pressure, liver problems, high blood sugar, enlargement of benign tumors of uterus.

Signs of rare side effects: breast lumps, unusual vaginal bleeding, dizziness and panting, speech changes, sever headaches, chest pain, shortness of breath, leg pains, vision changes, vomiting, yellowing of skin, eyes and nails.

If you notice any of the above signs tell your health care provider immediately.

A very serious allergic reaction to this drug is unlikely. If it does happen get emergency medical help if you have any of these signs of an allergic reaction. This may not be a complete list of side effects and may be subject to change, consult with your doctor or pharmacist if you are experiencing any worrisome side effects.

Overdose
If you suspect overdose, contact your local poison control center or emergency room immediately.
